If we can't help, they will be referred to a Veteran Service Officer (a VA-accredited advocate) or other community resource.

Become a PAID CAREGIVER for a family member in their home. 🏠
The Structured Family Caregiving program (SFC) with PROCLAIM HOME CARE offers financial and personal support services to seniors and/or disabled clients in their homes. The SFC program is a Medicaid Waiver program that pays a family member to take care of their loved ones that are elderly and/or disabled. Please feel free to contact our office to discuss available services and to send a referral for approval for the program.
SFC requirements to be eligible for financial compensation:
* You must live with the person receiving services.
* You are related biologically OR through marriage.
* The person you take care of is Georgia Medicaid-eligible and agrees to participate in the CCSP/SOURCE SFC programs. If needed, we can send referral to be approved for services of the program.
