Getting Started - Intake
To help our care managers get to know you and your family, we start by asking the following questions:
What are the challenges to your current caregiving situation?
How have these challenges affected you? Be specific.
Are there any other family members who are assisting you in caregiving your loved one?
Please list the health issues that your loved one is experiencing.
Are there activities that your loved one enjoys?
What do you need help with?
Your Loved One - Assessment
A complete head to toe physical assessment from a trained, licensed, registered nurse, which includes the following:
Cardiovascular
Respiratory
Skin
Digestive
Neurological
Safety
Mobility
Current Medications
Allergies
Caregiver
Personal Care Plan
A skilled nurse case manager will walk along side you, and your loved ones to develop a detailed plan to address all aspects of physical, mental, emotional, and spiritual health.
The care plan provides direction for the individualized care of the client/patient and is a means of communicating between the client, caregiver and the nurse care manager. Needs are addressed and the plans are updated as needed and as goals and circumstances change.
The care plan uses input from other disciplines like other disciplines like a licensed clinical social worker, chaplain, etc. to help problem solve and provide resources. Goals are established with the input of the client, caregiver and family.
What’s Next - Follow-Up
Your personal nurse care manager will be in touch with you to proceed with the assessment, and begin your journey to help at your fingertips. Your nurse will follow up as often as you desire, weekly, monthly, bimonthly. Care manager will help you with questions you may have like, what happens when? How do I? Where did I go to? What will it look like? How will I know when? How long?
