Aunkia Fowler and Associates Case Management AF & Associates delivers case management services for all three Medicaid Waivers (DD, CCW, and CMHW). We provide services in multiple counties in Wyoming.

At AF & Associates we are committed to improving the lives of the people we serve. We have over 20 years of experience in Wyoming Medicaid waivers and we are always working to expand our education to better serve our families. Please see below for additional information about our services. CHILDREN’S MENTAL HEALTH WAIVER:
High-Fidelity Wraparound is a planning process that brings people together from different areas of a family’s life to form a team. The team creates steps to help youth stay in their homes, schools, and communities. We coordinate are and build teams of support through the Wraparound model to manage day-to-day challenges of children, adolescents, and their families. Eligibility Criteria:
-Child aged 4 to 21
-Current diagnosis of emotional, mental, or behavioral disorder
-Medicaid eligible
-CASII score between 20 and 27

DEVELOPMENTAL DISABILITIES WAIVER:
The Developmental Disabilities Waiver is set up to assist individuals or families where one or more people have a lifelong disability that greatly effects daily life and requires more assistance in completing activities of daily living. When applying for this waiver, a participant will get a psychological evaluation and an ICAP completed, which will be paid for by the waiver to help determine eligibility. If the participant qualifies, the waiver will provide services both in the home and in the community. Eligibility Criteria:
-Aged 0 to 99
-Have a physical or developmental delay
-Resident of Wyoming

COMMUNITY CHOICES WAIVER:
The Community Choices Waiver is for individuals aged 65 or older, or an adult 19 to 64 years old with a disability. This program provides eligible individuals an alternative to nursing facility care through access to an array of community-based services. Eligibility Criteria:
-Aged 65 or older
OR
-19 years or older with a disability
-Resident of Wyoming
-Meet level of care determination through LT-101

Natrona County School District would like to assure families that the Free and Reduced-Price Meals program will continue for those who qualify during the government shutdown.

All students who currently receive free or reduced-price meals will continue to do so without interruption.

If your financial situation has changed due to the government shutdown, you may qualify for Free and Reduced-Price Meals. Applications can be found online at www.natronaschools.org, or a paper application is available at your child’s school. Families may apply or reapply at any time throughout the school year.

Below is a list of some community resources available. This list is not exhaustive of all the wonderful agencies, churches, and organizations in our community that provide resources for families. Please use the Wyoming 2-1-1 information or call 2-1-1 for a full list of available local resources.

Wyoming's Home and Community-Based Services (HCBS) waiver system is fragile. On September 1, 2025, providers statewide faced a 5% overall decrease in reimbursement rates.

The nitty-gritty:
• The HCBS waiver is a Medicaid waiver that allows states to provide services and supports to individuals in their communities.

• The WY Department of Health is statutorily required to commission a rate study every 2 to 4 years to establish a cost-based reimbursement system to pay for HCBS.

• The recent rate study conducted by Guidehouse reflects actual service delivery costs. HCBS providers are asking the Wyoming Legislature to fully fund the actual cost demonstrated in the study.

• Without this funding, providers are unable to pay competitive wages for direct care staff, providers are unable to accept people off the waitlist, and people risk losing essential services.
