Hospice care in rural Wyoming looks different than it does in bigger cities. And we wouldn't have it any other way.

Our team drives to Wheatland, Douglas, Glenrock, and everywhere in between across Natrona, Converse, and Platte counties. We bring care to ranch houses, small-town homes, and places where the nearest hospital is 45 minutes away.

Because where you live shouldn't determine whether you get compassionate end-of-life care. Whether you're in Casper or on a ranch outside Guernsey, our team comes to you.

We know these roads. We know this community. And we're honored to serve the people who call rural Wyoming home.

Serving Natrona, Converse, and Platte Counties with care that comes to you.

Meet Kilty Brown, Executive Director of Central Wyoming Hospice & Transitions.

Kilty didn't just find her way to hospice care, she found her calling. After years caring for patients across every stage of life, it was in those final, sacred moments that everything clicked. Providing comfort in a person's final days is one of the greatest honors of her life.

Thank you, Kilty, for leading with your whole heart.

We're lucky to have her. 💜

Sometimes you just need to feel the grass between your toes...and Jane's toes did!! She enjoyed a picnic with her son Jim complete with chocolate cake! 🥰🥰🥰
She wrote the sweetest note:
"Dear Staff,
Thank you for arranging the picnic. It was more than I could have hoped for!
We laughed and hugged and I got a picture of my feet in the green grass. We enjoyed the wind, clouds, and sun all in one afternoon.
Thank you,
Jane"
This is what hospice care looks like. ❤️❤️❤️

Grief looks different for children.

Sometimes it shows up in quiet moments. Sometimes in big emotions. Sometimes in ways that are hard to put into words.

📍 Ropes & Roses, Casper

This camp is open to youth ages 6–16 who have experienced the loss of a family member or friend within the last three years. Thanks to the generosity of the Gertrude Kamps Memorial Foundation, there is no cost to attend.

Easter is a season of renewal.

But renewal does not always mean something new.
Sometimes it means finding a new way to hold what has always mattered.

A memory.
A relationship.
A love that does not go away.

At Central Wyoming Hospice & Transitions, we see that every day, in the quiet strength of the families we serve.

This season, we honor the way love continues, even through loss.

With you. 💜

You're facing a serious illness. You're still seeking treatment. But the daily challenges are piling up.

Our Transitions program offers free, non-medical support to help you stay independent: companionship, transportation to appointments, help with errands and correspondence, respite for your caregivers, and connection to community resources.

Since 2005, we've matched people with case managers and volunteers who understand what you need—whether that's a ride, help with daily tasks, or just someone who gets it.

You don't need to be enrolled in hospice. You don't need to pay. You just need to reach out.

After a long hospital stay, Kris faced the most difficult decision of her life.

One call to Central Wyoming Hospice & Transitions brought guidance, compassion, and a steady presence during a week filled with uncertainty and love. Her husband’s time in hospice care was less than a day, but in those hours her family experienced respect, privacy, and sincere care that allowed them to say goodbye with peace.

In the months that followed, the support continued with notes, grief guidance, and the reminder that they had not been forgotten.

We are honored to walk alongside families in moments that matter most.

Our Story

Whatever challenges your family is facing with a serious illness, we are here to offer support, expert care, and understanding. Our goal is your comfort, quality of life, and peace of mind.

In 1981, Central Wyoming Hospice began as a Wyoming-grown, volunteer organization to help families in our community facing the challenges of end-of life. Since then, as the only non-profit hospice in Natrona and Converse counties, we continue to be committed to helping our neighbors have the best days possible.
