New news.

MRIs of healthy old shoulders will show degeneration of the rotator cuff. Like partial thickness tearing. However, those people didn't even have pain.

To sum it up. The MRIs definitely help ID anatomical deficits, but it doesn't mean we do surgery to correct something not bothering us.

I have definitely sent people to surgeons after stalling out on further improvements with conservative therapy and alternatives.

Easy assessment for older patients with sudden confusion and recent sickness. This is a common tool in ICU settings.

Treatment for delirium begins with symptom recognition. Reorienting patients frequently and providing daytime stimulation through exercise or outdoor activities is helpful. Sunlight and limiting naps during the day can be beneficial. Reducing background noise and creating a calm environment at night are also key.

What is delirium?

A sudden, temporary state of severe confusion, disorientation, and reduced awareness, often caused by underlying medical issues like infections, medication side effects, or organ failure. It typically manifests with rapid, fluctuating changes in mental state, memory, or behavior.

Try the CAM ICU test to rule out delirium if your loved one is experiencing this.

The more you know.

Red Flags for Cancer.

I've got over 19 years experience of being a physical therapist. I've been an ICU therapist for a long part of that. I've seen cancer. I've seen the signs and symptoms.

I saw some of these red flags with a client and referred back to the physician to rule out what I hoped was just a spine issue. It was more than that. It was cancer.

Tuesday recap of an Acute Ankle Inversion Sprain.

If you need the cliff notes, then here it is.

1. Can I weight bear on the injury = helps rule out a fracture

2. Treat it with PEACE.

3. Protect the injury : crutches and a CAM boot
4. Elevate to limit the swelling cycle
5. Avoid NSAIDs to promote healing
6. Compression of the joint with ace wrap/wool sock
7. Educate: keep it moving in a pain free way

Sorry laying on the couch for 2 days with ice and anti inflammatory medicine is not the way any more.

It's feeling so much better 36hrs later. My bruising is showing up and the swelling has settled. I'll wear the CAM boot for 1 to 2 days to protect it until I feel comfortable enough to wear a regular shoe.
