✨This is what the Social Care Network (SCN) Program looks like in action — connecting Medicaid members to essential supports like food, housing, and home health services.✨

🏠 Submitted a Unite Us referral to help a family secure the majority of funds needed for a housing security deposit before their move.
🩺 Coordinated and implemented home health services to ensure a family had the in-home support and continuity of care they needed.
🥗 Secured and coordinated fresh food or medically tailored meal deliveries for 16 individuals — with weekly deliveries scheduled over a six-month period

If you know someone with Medicaid who could benefit, have them complete an intake form on our website. Sara or Jen will reach out directly. 🤝

🕊️ Meet the compassionate faces behind our End-of-Life Doula Program.

Our doulas walk alongside individuals and families during one of life’s most tender seasons — offering steady presence, open conversations about dying, emotional support, guidance through the signs of the dying process, and practical assistance to ease the load on caregivers.

They help create space for reflection, legacy work, and meaningful discussions about advance care planning — ensuring no one feels alone. 🤍

We are honored to offer End-of-Life Doula services to residents of Chenango County, Madison County, and Oneida County at no cost.

If you’d like to learn more about how our team can support you or a loved one, please reach out.
