I'm at the AAU junior olympics with one of my athletes who recently started complaining of ankle pains. This led me to remind you all that ankle sprains are some of the most common training injuries. They are more likely to occur from plantar flexion (which is essentially the end position of your feet after calf raise) or after inversion of your ankle (almost like trying to stand on the lateral or outer side of your foot). Remember RICE- rest, ice, compression and elevation.

Strength training is an important part of any good work out program. However, there are countless times when novice and seasoned lifters find themselves in my emergency room with rhabdomyolysis. It's a fancy word for the fact that the muscles are dangerously breaking down. Some of the most common symptoms are cramping, especially in the muscle group you focused on, and possibly dark urine. This condition becomes more likely to occur if you're not well hydrated or if you work out in very hot temperatures.

Trainers, be aware that some clients have medical conditions that affect their digestive track. You must not minimize it if a client tells you that their condition is flaring up. One such example is a Crohn's Disease flare. Often these people will have abdominal pain and fatigue, among other symptoms, that will likely challenge their work out performance.

When training in the heat be mindful of the dangers of training in high temperatures. Some common symptoms of heat exhaustion are headaches, general weakness, lightheadedness, and nausea. If a client starts complaining of these symptoms then resting in a cool setting with fluid and electrolyte replacement is the appropriate treatment. When I treat patients in the emergency room with heat stroke (which is the extreme of heat injuries) I use a water mist and fan in addition to cold packs to rapidly cool people. You may consider the these measures for heat exhaustion as well.

Hey fitness friends. Here's a droplet of fitness knowledge from Dr. J:

A well planned training session should consider a client's underlying medical conditions and medications. For example, some patients have abnormal heart beats or high blood pressure. These patients may be on medications like beta blockers and calcium channel blockers.
These medications, though effective, may affect a person's heart rate in addition to blood pressure.

This should be considered when determining a client's target heart rate. It should also be considered when monitoring a client's response to a work out. If the client complains of lightheadedness, take this very seriously. This may be the first sign of imminent syncope; which is when one collapses and loses consciousness.

Hey fitness friends. Here's a droplet of fitness knowledge from Dr. J:

If you have asthma and are planning to exercise, then you should be mindful of your triggers and prepare before your training session begins. For example, if your trigger is seasonal allergies, then you should take your daily allergy medicine. Discuss your asthma maintenance with your physician accordingly.

Also, be sure to bring your rescue inhaler to your training session in the event of an asthma attack.

Trainers, most clients who have ever experienced an asthma attack know what it feels like. Trust them if they tell you that they need their inhalers during the training session. Unfortunately, the signs of an asthma attack are similar to what you may see after a training session with a non-asthmatic; i.e. fast breathing or labored speech.
