If it looks and smells like chronic mesh infection, it must be it. Acceptance is tough, so we every now and then see patients who underwent index repairs at other institutions.
Some mesh infections can be salvageable. Some are not.
There are any risk factors of surgical infection after hernia repairs: smoking, poorly controlled diabetes, immunosuppression, contamination during surgery…

Dr. Brook Learned of Division of Endocrinology giving Surgical Grand Rounds on perioperative management of diabetes.
Poorly controlled diabetes and post-operative glucose levels are associated with higher morbidity including surgical infections. Careful control of blood glucose is one of the three cornerstones of elective hernia repair. We should aim for pre-operative level of HbA1c not more than 7%. Most complex hernias can wait, so better to invest some extra time in getting glucose under control.

Durable minimally-invasive repair of groin (inguinal, femoral) hernia is based on wide overlap of mesh over all necessary clearly visible structures, so called “critical view”. Most recurrences we see are caused by mesh of insufficient size or incomplete dissection. Minimally-invasive repair allows to cover all potential weak spot in the groin. Here is an example of “pantaloon” hernia with defects in direct and indirect spaces. Critical view of inguinal hernia repair was suggested by prof. Edward Felix and Jorge Daes in 2017.

One of the common hernia repairs is based on mesh placed inside the abdominal cavity, so called IPUM (IPOM). This technique is technically simple and requires special meshes with an anti adhesive coating, like on non-stick pan. The problem is that intraabdominal mesh often cause extensive adhesions (scar tissue) between bowel and the mesh. Recently I did a robotic repair for recurrent incisional hernia in a patient who had several previous intraabdominal meshes. It took me several hours to safely divided all the adhesions so we can perform robotic TAR procedure and reliably repair the hernia. However, carefully performed IPUM repair is still a viable option, particularly for small hernia when peritoneum is very thin and friable.
