Tom experienced a stroke in July 2024. After months of hard work in rehab, he made a decision: he was going to keep moving forward.

In January, we introduced Tim through a mentor shift, and it didn’t take long to see it was the right fit. Once physical therapy wrapped up, they built a new routine together, mornings at the YMCA, healthier meals, and regular rides on the Lincoln Prairie Trail.

What started as recovery has turned into momentum. Biking, strength work, protein-packed lunches, and structure at the start of every shift have given Tom renewed energy and purpose.

Stroke recovery doesn’t end when therapy does. Ongoing movement, nutrition, and consistent support are often what help rebuild confidence and independence long term.

This is what personalized care can look like.

Join us on June 23, 2026 at the Illinois Assistive Technology Program - IATP for a morning focused on education, resources, and connection.

From understanding Alzheimer’s & dementia behaviors to protecting seniors from scams and learning about essential legal documents, this event brings together experts dedicated to supporting older adults and their families.

SYNERGY HomeCare will also be sharing community resources alongside Advanced Healthcare and Transitions Hospice — because knowing what’s available can make all the difference when needs change.

It was a great morning with the American Business Club of Springfield!

Taylor had the opportunity to share how SYNERGY HomeCare can serve as a trusted resource for individuals, families, and community partners. The room was filled with active, engaged members who care deeply about supporting our community — and that energy was contagious.

When business leaders understand the signs that someone may need extra support at home — such as changes in routine, increased fall risk, or caregiver stress — they can help connect families to resources sooner. Early conversations often lead to safer outcomes and greater independence.

💐✨ A Beautiful Galentine’s Afternoon ✨💐

We had the sweetest time celebrating Galentine’s Day at Springfield Suites! 💕

Advanced Healthcare, Transitions Hospice, and SYNERGY HomeCare partnered together to host a special afternoon for the Springfield Suites staff and community partners. We built gorgeous flower bouquets from Flourish, enjoyed delicious snacks, sipped festive mocktails, and most importantly — spent quality time connecting.

Events like this do more than brighten a day. Strong professional relationships help create smoother care coordination, better communication, and stronger support systems for the individuals and families we serve.

Grateful for collaboration, community, and a little extra sparkle in the middle of winter. 🌸💜

A very special congratulations to Wilma & Butch for being crowned King & Queen of the Valentine’s Party! 👑 What a beautiful reminder that love stories don’t have an expiration date.
