Dear patients,

We hope you are doing well. We are writing to inform you that Dr. Kathryn Donesa-Zuzak has decided to relocate to another state for family reasons. Her last day in the clinic at Live Well Family Medicine will be December 30, 2022. Dr. Donesa-Zuzak has been a valued member of our team and we will miss her dearly. We wish her the very best in her new chapter.

We are committed to continuing to provide you with the excellent care you have received up until now. Should you wish to continue with our practice we will assign you a new P*P. You can still book your appointments at 480-800-3561 and get the medical care you need. Do not hesitate to call us with any questions regarding the transition.

If you wish to transfer care to another physician in the community please let us know as soon as possible. You will need to sign an authorization form so we may transfer your records to your new doctor. Original records will be retained by Live Well Family Medicine.

Dear Live Well Family Medicine Patient,

We hope you are staying well and finding solace and calm as you shelter in place which is our best solution to staving off the coronavirus menace.
Please know that we are available to address ALL your health needs through a telemedicine service. We can evaluate you through a video conference. If you need to be seen in person, we will arrange a way to see you safely. Our office is open from 8 am to 5 pm, Monday to Friday, BUT please call us before you come in so we can decide how to best handle your concern.
Our most important advice: stay home, wash your hands, avoid touching your face, cover your face when in public, and observe social distancing. This is an opportunity to focus on your inner self, meditate, exercise, tackle the project you have been putting off, stay connected and communicate with loved ones, and remain hopeful that we will pull through this. We are all in this together.
