Please fax us a copy of the prescription of your order and demographics for the patient, including insurance information and a copy of the sleep study if ordering a CPAP or BiPAP). If you have a STAT request, please let us know and we will get it done! If we cannot take the insurance of your referral, we will forward your request to the correct provider and advise you."
