We are a home health care provider focused on delivering skilled nursing and supportive care that allows patients to receive quality medical services in the comfort and safety of their own home. Five Star Home Health Care is a full-service home health agency, providing comprehensive medical and non-medical care in the home. Our clinical services include skilled nursing, advanced wound care, obesity treatment, and a full range of therapy services for individuals recovering from surgery, illness, injury, stroke, or other medical conditions. We also provide compassionate Home Health Aides for live-in, overnight, and daytime care. Aide services may include assistance with personal hygiene, meal preparation, medication reminders, companionship, and transportation to appointments or social activities. Nearly half of all patients discharged from the hospital require additional care, treatment, or rehabilitation. Too often that care is fragmented between multiple providers, leaving patients and their families overwhelmed. At Five Star Home Health Care, we simplify the process by integrating all home health services under one coordinated team. This whole-person approach ensures smoother care transitions, better communication, and greater peace of mind for our clients and their families. Our team manages the transition from hospital to home and works to reduce unnecessary hospital readmissions. We do not rely on centralized call centers, allowing us to provide responsive, personalized service. The Medicare GUIDE program offers comprehensive support to family caregivers of individuals living with dementia:

Education and Training: Family caregivers receive specialized training on managing dementia-related behaviors and providing effective care. This equips them with the knowledge and skills to feel more confident and capable in their caregiving role.

Emotional Support: Through counseling and support groups, GUIDE offers caregivers essential emotional support to help them navigate the challenges and stresses of caring for a loved one with dementia.

Respite Care: Caregivers have access to respite services, allowing them to take temporary breaks from their caregiving duties. This much-needed relief helps them recharge, which is crucial for their overall well-being.

Care Navigation: GUIDE assists caregivers in coordinating medical and social services, reducing the complexity of managing multiple appointments and care needs.

Resource Access: Caregivers are connected to community resources, such as home health aides and adult day care programs, providing them with additional support to enhance care for their loved ones.

Our Story

Five Star Home Health Care makes tomorrow a better day. Seniors and individuals with disabilities prefer to live in the comfort of their own home near family, friends and pets. We ensure that our clients are safe, comfortable and thriving in their own home by providing the services necessary to make independence at home a reality. Our clinicians provide treatment after an accident, surgery, illness or other disabling event. We also provide assistance with activities of daily living on an hourly, overnight or live-in basis.

We provide the following professional services in the home: Skilled Nursing, Physical Therapy, Occupational Therapy, Speech Therapy, Medical Social Worker, Home Health Aide, Registered Dietitian, Behavioral Health and Exercise Physiologist.

Our specialty programs include: Advanced Wound Care with MIST Therapy, Parkinson's care with LSVT Big & Loud, Speak Out!, VitalStim, orthopedic pre-hab and rehab, dementia screening with BCAT tools and our Honest Medical Weight Loss service for obesity. We treat chronic diseases such as CHF, COPD, diabetes and infections.

Half of all individuals discharged from the hospital require additional care, treatment or rehabilitation. They want to go directly home - not to a nursing facility. The problem is that care is often poorly coordinated and it’s divided among many different providers. We make the process smoother and less worrisome by integrating home health services from one company. We are preferred by clients, hospitals and payers with our fully integrated post-acute care solution.
