Continuity of care is one of the most important parts of mental health recovery.

For providers across Charlotte, the NAMI Navigator Charlotte Directory has become a trusted tool to ensure individuals and families can find the support they need after treatment.

“At Guidelight, as a new provider in the Charlotte area, the NAMI Navigator program has quickly become an invaluable resource for both our clinical team and the clients we serve. We rely on the Navigator directory routinely as part of our discharge planning process to ensure clients have seamless continuity of care.

The tool allows our team to efficiently identify trusted community partners, mental health providers, and support services that align with each client’s unique needs. Whether we’re helping someone transition to outpatient therapy, connecting a family to specialized support, or locating additional community-based resources, the NAMI Navigator directory provides a clear, accessible pathway to the right level of care.”

— Alexa Eischeid, Director of Clinical Outreach – Charlotte, .Health

Partnerships like these strengthen Charlotte’s mental health ecosystem and help ensure that no one leaves treatment without a supportive next step.

In 2025, we deepened our commitment to equity and visibility — ensuring mental health support is a promise, not a privilege.

Through bilingual services, culturally responsive programming, and community partnerships, we expanded who feels seen, heard, and supported.

Equity is not an add-on. It is foundational.
