So grateful for leaders like Matt, our Discharge Director, and Desiree, our Discharge Planning Assistant, whose dedication, compassion, and teamwork make such a meaningful impact every day.
They took time to step away and enjoy a well-deserved team outing at Texas Roadhouse—great food and even better company!
We are truly thankful to have them as part of a team that supports one another and the patients we serve.

At Charlottesville Health & Rehabilitation Center, we’re one of the largest rehabilitation centers in the area, and we’re proud of our center and the exceptional care we provide. We offer newly remodeled rooms and rehab gyms that are being constantly updated with the newest and most modern equipment. Our center underwent a complete renovation and has been transformed with beautiful modern amenities and private rooms.

Charlottesville Health & Rehabilitation Center is an active part of the local community. We’re involved in the Alzheimer’s Association and the Walk to End Alzheimer’s and also participate in the annual MS Walk and, most recently, a Recycling Program through Ronald McDonald House Charities. We serve Charlottesville, Albemarle County, Louisa, Crozet, and beyond.
