Jason visited our office a few months ago and could barely walk through the door. He reported a 10/10 pain in his lower back/hips and after 24 hours he was down to a 4/10. One of the coolest things about Jason is that he is someone who knows a bit about everything. On that second visit he was so excited about feeling better he quoted this Shakespeare soliloquy from Henry IV:

We are now offering CDL Physicals/DOT exams. We will offer same day appointments. Call ahead preferred, but walk-in appointments are available.

You will need to bring the following to your exam:

-Driver’s license
-Glasses (if you need them)
-Hearing aids (if you require them)
-You will have to provide a urine sample so be prepared
-Complete medication list with dosages
-If you have special medical exemptions or recent medical approvals you will need to bring these with you
