She had been diagnosed with bipolar disorder, depression, anxiety, and ADHD.

Years of treatment. Multiple medications.
And still… nothing really worked.

When we looked at her brain, a different story emerged.

Her qEEG showed significantly elevated alpha activity (8–10 Hz) across the brain, a pattern often associated with underactivation, low mental energy, and difficulty engaging.

This can show up as:
• Brain fog
• Low motivation
• Trouble starting or completing tasks
• Emotional disconnection

On the surface, those symptoms can look like multiple psychiatric conditions.

But in some cases, the deeper issue is how the brain is regulating itself.

If you only treat the symptoms, you can miss the pattern driving them.

Part 2: Treatment implications

This patient was recently started on Prozac after not tolerating Effexor, but the bigger treatment question here isn’t medication.

On qEEG, this profile shows underactivation and compensatory relative power shifts, not hyperarousal.
That changes the treatment target.

Neurofeedback for this pattern focuses on increasing activation, improving efficiency of alpha and low-to-mid beta, and correcting frontal asymmetries linked to withdrawal and low motivation.

The goal isn’t to calm the brain down.
It’s to help it generate enough signal to support engagement, energy, and follow-through.

Medication can support stability, but self-regulation training is what helps retrain the underlying patterns.

Let’s review this qEEG.
This adult male presents with significant depression, anhedonia, and low energy.

You’ll notice relative power elevations along midline regions (Pz, Cz, Fz, Fp1).
Importantly, these are not excessive in absolute power.

When relative power is high but absolute power is normal, the brain is often compensating, not overactive.
The issue here isn’t anxiety or hyperarousal.
It’s underactivation of networks that support motivation, engagement, and drive.

This is why some forms of depression feel flat rather than distressed
and why treatment needs to focus on restoring activation, not just suppressing symptoms.
