Our partners at Humboldt Park Health are hosting their 2nd Annual "Girl, let me tell YOU!" comedy fundraiser! This event is a fantastic way to support Breast Cancer Awareness Month and equitable care.

🗓️ When: Thursday, October 23rd • 7:00 PM 📍 Where: Joe's Bar (940 W. W**d St., Chicago)
100% of the proceeds support breast cancer screenings and education. In fact, every two tickets sold funds a mammogram screening for a woman in need. We applaud Humboldt Park Health for their dedication to advanced screening technology and equitable care in our community!

Jeffrey's story is a powerful reminder of how comprehensive, compassionate care can change a life.
After battling depression and substance abuse, Jeffrey met Community Health Worker Teresa Guillen. Teresa connected him to primary care, mental health counseling, housing, transportation, food, and education assistance truly addressing every social driver of health.
Today, Jeffrey is sober, his depression is controlled, and he's in school to become an RN. As he says, "Teresa restored my hope."
This is the power of our partnerships and the dedication of our CHWs!

La historia de Jeffrey es un poderoso recordatorio de cómo la atención integral y compasiva puede cambiar una vida.

Después de luchar contra la depresión y el abuso de sustancias, Jeffrey conoció a la promotora de la salud comunitaria Teresa Guillén. Teresa lo conectó con un medico, consejería de salud mental, vivienda, transporte, alimentos y asistencia educativa, abordando verdaderamente todos los determinantes sociales de la salud.

Hoy, Jeffrey está sobrio, su depresión está controlada y está estudiando para convertirse en enfermero (RN). Como él mismo dice: “Teresa me devolvió la esperanza”.
