I have an amazing team of leaders pulling together the final post course modules for The Primary Care PT Start-Up Workshop for Business Leaders.

I'm coming in to talk about the people things - not just clinical competencies, but team dynamics, and values.

Too many 'courses' focus on information, perspective and knowledge, but not on ACTION. That's just now how I do things. The ONLY thing that matters is the ACTION. This workshop turns on the action for those who are ready to dive in.

The outpatient PT industry needs a revolution. That's no secret.

In the current model, we're not effectively demonstrating high value, cost containment nor solutions for managing populations at scale. In fact... most are following a cookie cutter model with concepts from the 80s that no longer meet the needs of our population.

The primary care PT model provides an opportunity for us to The outpatient PT industry needs a revolution. That's no secret.

In the current model, we're not effectively demonstrating high value, cost containment nor solutions for managing populations at scale. In fact... most are following a cookie cutter model with concepts from the 80s that isn't engaging with 90% of the MSK population.

NEW PAYMENT MODELS...

What is the patient's repsonsibility here?

With MSK pain and functional complaints, we know that patients have much better outcomes if they have better plan adherence, which ultimately include lifestyle and behavior modification. For those of you that have ever tried to modify a behavior or add in a new habit, like an exercise routine that you've never really consistently done before..... It is really really hard!

It requires a lot of motivation and a lot of support.

With these new Medicare payment models, the idea in the goal is that improved longitudinal digital support will improve outcomes. But what about patient engagement?

Life is really busy. Everyone reading this post selectively chooses what tasks to include and what emails to actually read based on our perception of priority.

So...what is the patients incentive to partake in ACCESS? Or really any longitudinal healthcare program?

Healthcare delivery markets are shifting...

The way we are currently doing things is inefficient, exponentially expensive, and creates extensive and entirely preventable backlogs(that's a lot of e-words...)

The outpatient PT model (the 2x/week for 8 weeks thing?) as we know it in 2026 will cease to exist in 10 years. In fact, that's true for a lot of outpatient healthcare.

The original trust fund that covered Medicare Part A is expected to be depleted between 2033-2040. This means that, in order for medicare beneficiaries (that's all of us Americans, now or in the future) to maintain Part A access, payroll taxes will likely increase. That ALSO means that Medicare is likely to explore lower cost, higher efficiency care delivery strategies... introducing - ACCESS, TEAM, REACH, ASM and other CMS innovation programs that will attempt to provide care via lower cost AI/tech enabled care delivery methods.

All this to say.... PTs need to start rethinking HOW we can leverage our skills to meet the physical/functional/MSK needs of patient populations at scale. And if you haven't been following... news flash -a lot of companies are already doing it!!!

MUSC MSK 1 lab 💪🏻

I am an privileged enough to be an adjunct faculty member in a few DPT programs. I spent this past week with MUSC hybrid DPT in Charleston helping with their MSK 1 lab intensive.

Here's a tiny recap of the week getting familiar with hip and knee exam and intervention, culminating in the students first MSK practical exam. They were so nervous, but their hard work and preparation paid off. 🙏

It's a CRAZY reminder of how steep the learning curve is. And how much knowledge is required to truly understand how to help people with complex health conditions.

It ain't easy! But these students did an awesome job 👏🏻

Last CSM Recap post...

This year really was different.

I've been to a lot of CSM conferences since I was a first year DPT student in 2012. In fact, this was my 10th yr attending. Prior to this year, I was starting to notice some concerning trends that made me want to invest my time and money elsewhere. I didn't even want to attend this year, but I was asked to contribute to several sessions - 2 of those were selected.

And the conference delivered. Drew Contreras called it out in a recent LinkedIn Post. There's an appreciable shift... we're in a pivotal moment in our professions history. And against all odds, people are starting to embrace that we NEED to pivot and getting excited about it.

These photos capture just a handful of colleagues that I was able to connect with!

And how fun to bump into my former neuro professor from Temple who carried the beaming glow of being newly retired! Congrats!

CSM recap 1... of many.

Special shoutout to my new friend Dr. Nica Jacobson. Nica came to my CSM session this past Saturday on Doctoring in Physical Therapy - she made a point to introduce herself after the session and the exchange brought tears to my eyes 🥹. While I can't remember the exact words, it was something along the lines of... "I had a feeling I should come to CSM this year but didn't know what I was looking for or why I had this feeling, but this was it. This is exactly what I want to do and where I want to go." She proceeded to thank me for all my work in this area and THAT was what I needed to hear. I needed her words just as much as she felt she needed mine....

Pioneering kind of sucks a lot of the time. Its lonely, confusing, often thankless and plagued by highs and lows - endless hope, followed by drowning doubt.

But this is why I'm here making waves - to inspire and to create a positive impact for the future. REAL positive, meaningful and sustainable change.

Bc healthcare right now sucks. It won't be there when we need it to be unless we put in the work...
