Effective October 11, 2022, Heartland Health Centers is now Tapestry 360 Health. Our mission is to improve the well-being of the communities we serve by providing accessible, high-quality healthcare. Our vision is to be a national leader in community-based health care by advancing innovative service models and patient-centered best practices. Tapestry 360 Health is a federally qualified health center with 18 locations serving Chicago’s north side and nearby suburbs. In 2020, we provided 27,000 patients with a medical home where we offer affordable and comprehensive primary care, oral health care, and mental health care services. As a medical home, Tapestry 360 Health ensures comprehensive, affordable, quality, safe, and coordinated health care that is culturally competent and orientated to addressing all the health care needs of each patient. We're recognized for delivering high-quality holistic health care, with awards from the U.S. We're accredited by the Joint Commission and certified as a Primary Care Medical Home. What is an FQHC? A Federally Qualified Health Center (FQHC) must serve an underserved area or population, offer a sliding fee scale, provide comprehensive services, have an ongoing quality assurance program, and have a governing board of directors. Language is access. Culture is care. Meet Mujibur Reahman, one of the incredible people behind our Refugee and Immigrant Care team 💙

Mujibur Reahman’s commitment to care was shaped long before he joined Tapestry 360 Health. After more than a decade of humanitarian work in Malaysia—supporting refugees, rescue efforts, and communities without access to healthcare—Mujibur now brings that lived experience to our Refugee and Immigrant Care team at our Devon location. His perspective isn’t something learned in training—it’s something he has lived, and it makes a powerful difference for the patients we serve.

As a Community Health Coordinator, Mujibur brings deep cultural insight and fluency in seven languages, helping patients feel seen, heard, and supported every step of the way. His work is a powerful reminder that equity begins with understanding—and community care starts with trust.
