ATW Health Solutions is hiring Community Health Navigators to support Medicaid redetermination and enrollment efforts across Cook County.

We are currently seeking both a Bilingual Community Health Navigator (Spanish/English) and a Community Health Navigator to work directly with individuals and families at community events, clinics, and outreach sites. These contractor roles play a critical part in helping residents maintain access to healthcare by guiding them through Medicaid redetermination, assisting with enrollment forms, and connecting them to Cook County Health services, including primary care, behavioral health, and preventive services.

This role is ideal for individuals with experience in community outreach, healthcare navigation, enrollment assistance, or social services who are passionate about working directly with communities and making a measurable impact. Navigators will engage with local partners such as churches, shelters, and community organizations and provide one-on-one support to help residents access essential healthcare resources.

Position highlights:
• Contractor role based in Chicago, IL, with outreach across Cook County
• $20 per hour, paid twice monthly
• Flexible schedule including evenings and weekends
• Direct community engagement and healthcare enrollment support
