This week (Sept. 12-18) is Direct Support Professionals Recognition Week – a week to appreciate and celebrate the hard work and contributions of DSPs who embrace & every day.

Direct Support Professionals (DSPs) are our front line staff working directly with individuals diagnosed with Developmental Disabilities or mental illness. One of the most important positions at Ohana Community Services, a DSP supports individuals with increasing the quality of life and ensuring growth and independence. Their commitment makes Ohana’s vital programs & services possible, and they have been during the pandemic.

Ohana Community Services, LLC is looking to hire a Registered Nurse (RN) Trainer to open and oversee Community Integrated Living Arrangements in the West side of Chicago Area. The Registered Nurse (RN) Trainer will be working with the developmentally/intellectually disabled population providing services allowing the individuals to live in their own home in the community.

This position is responsible for:
providing employee medication training
completing assessments
auditing and managing medical charts
manage medications for the individuals
Responding to all calls, emails, and message to give appropriate medical/nursing directions

Additional duties include:
getting monthly MARS (Medical Administration Record) ready and available for all homes
Ensuring all record keeping and/or required documentation is both accurate and current
Completing and distributing medication error reports as appropriate.

Requirements:
We are looking for a candidate that is looking for an opportunity to oversee and provide exceptional care to adults with developmental/intellectual disabilities. The candidate must be a good communicator, highly organized and work well unsupervised. Travel is required with reimbursement.

Current License from the State of Illinois as a Registered Nurse
RN trainer certification for Med Adm Rule 116 preferred, will be provided if do not have
Valid driver’s license and access to a reliable vehicle

Ohana Community Services, LLC. will be providing 24-hour support for all individuals who we are serving effective March 12, 2020, through April 15th, with the possibility for expansion past this time as necessary. As we all work through this challenging time together we want to reach out and share our plans at Ohana Community Services, LLC; Individuals will have their temperature taken and recorded each twice a day; as PRN. Community Outings are limited to an area with low activities. Individuals will have the choice to pick activities of their liking with limited due to the COVID-19. More in-house activities, games, are encouraged.

Any staff member who is not feeling well is asked to stay home, return with a doctor's clearance. Upon arrival, all staff has their temperature taken and recorded and completes a brief survey to ensure that they are fit for work. We have reviewed our employee roster we are confident that at this time we will have the necessary coverage to provide our services without disruption until April. However, we are requesting for temporary Direct support professional (DSP) to apply for part-time in case of an emergency and/or staff shortages.

Assist in the coordination of direct care services provided and ensure implementation of

homes.
MAJOR DUTIES AND RESPONSIBILITIES
1. Assure that the homes are in compliance with Federal, State and local standards,
guidelines, and regulations for all programmatic and direct care services.
2. Assist in the coordination of the efforts of the Community Support Team and program
staff to ensure that each individual of each home is receiving optimum services geared to his/her individual needs.
a. Shall assist in the completion of annual assessments on CILA clients for the purpose of program planning. Annual Assessments shall include obtaining other assessments, i.e. medical, dental, psychological, physical therapy, SAMA, etc.
b. Shall assist in the development and coordination of individual program plans for each CILA client.
c. Shall act cooperatively with day program staff, consultants, and external agencies
(PAS, etc.)
3. Ensure that individual client’s programs meet their needs and are provided on a continuing basis.
4. Assist to develop, implement, document and update specific planned training programs.
5. Assist to coordinate all referrals, admissions, transfers and discharges of individuals from the facility.
6. Assist in the implementation, maintenance and coordination of a filing system for each
individual.
7. In conjunction with the QIDP / Executive Director, interview, hire, schedule, evaluate and terminate program staff.
8. Assist CILA clients in individual budgeting processes.

JOB DESCRIPTION
House Manager
SUMMARY
The House Manager assists in the coordination of direct care services provided and ensures implementation of active treatment as designed in the individual habilitation plan for individuals within the homes.
EXPECTATIONS
The House Manager utilizes the time-clock and will be at the CILA homes a minimum of 35-40 hours each week. Any deviations below 35 hours need to be sent to the QIDP.
ESSENTIAL DUTIES AND RESPONSIBILITIES are as follows, but other duties may be assigned:
1. Assist the QIDP in maintaining compliance with Federal, State and local standards, guidelines, and regulations over your respective home(s).
2. Provide input to the Community Support Team regarding the residents of your home(s).
3. Provide input to the QIDP, Supervisor, BCBA, and RN for completion of any required assessments. 4. Schedule and ensure transportation to all necessary appointments.
5. Assist in the implementation, maintenance and coordination of a filing system under the guidance of the QIDP.
6. Seek qualified applicants to fill any needed staffing vacancies. (QIDP will conduct all interviews.) 7. Assist CILA individuals in individual budgeting processes and maintain resident checking accounts. Balance accuracy needs to be compared to their online banking statements or physical banking statements. 8. Calculate cost of care for individuals and assist all individuals in writing current cost of care checks and submit to the main office.
9. Provide documentation as required.
10. Submit requisitions for materials, equipment, etc. as needed. All purchases over $500 must be run through the main office. All purchases over $100 must be first approved by your QIDP or Supervisor.
11. Attend all in-service sessions, workshops, and conferences as required.
12. Keep confidential all information concerning individuals.
13. Complete other tasks as deemed necessary and appropriate within training and capabilities, or as may be directed by your Supervisor or QIDP.
14. Coordinate purchasing and repurchasing of incontinence supplies through Medicaid approved vendor. 15. Willing to work beyond normal work hours, and other positions temporarily when necessary. 16. Assist in training new personnel regarding the CILA homes, its policies, procedures, individuals and to his/her job position and expectations at the home.
17. Conduct job training specific to the individual(s) of your home as delegated by the QIDP.
18. Manage the scheduling for the staff at the CILA site. Ensure that all overtime is a last resort measure and receives prior approval from QIDP.
19. Conduct any delegations made by the RN.
20. Assist in observing and training new staff in passing medications as an additional quality assurance tool beyond what is completed by the RN.
21. Ensure all ongoing protective maintenance issues for the house are being conducted: oil changes are done quarterly, furnace filters are changed quarterly, leaky toilets are fixed promptly, and gutters are cleaned semiannually.
22. Conduct monthly emergency drills and maintain documentation
23. Schedule, plan and ensure resident money is available for activities.
24. Ensure that monthly resident council meetings are being completed.
25. Conduct grocery shopping and facilitate the menus using nutritional needs and resident preferences. 26. Ensure cleanliness and maintenance of home(s)
27. Monitor individual appearance of persons supported in the homes by ensuring that each person is dressed and groomed appropriately.
28. Reconcile credit card bill monthly.
QUALIFICATIONS
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. In addition, the individual must pass a state background check. EDUCATION AND/OR EXPERIENCE
The house manager must possess at least a high school diploma and a valid driver’s license. Some advanced education is preferred. He/she must be computer literate and be able to competently complete information through the use of Microsoft Word, Microsoft Works, Microsoft Excel, e-mail, and the internet.
LANGUAGE SKILLS

He/she must be able to effectively present information and respond to questions from outside agencies and the general public.
