Chicago Northside Collaborative, a Healthcare Transformation Collaborative funded by the Illinois Department of Healthcare and Family Services, is a partnership with a common goal of improving access to specialty care and services and addressing health care disparities that impact the communities in the Northside of Chicago. Endeavor Health Swedish Hospital , together with the five collaborating FQHCs: Erie Family Health Centers, Heartland Health Centers, Hamdard Healthcare, AHS Family Health Center, and Howard Brown Health, are embedding specialists at FQHCs and have onboarded additional specialists to increase access to care and overall to continue providing comprehensive, high-quality, and abundant access to primary care for their community.

The Chicago Northside Collaborative has successfully reduced ED visits by 52%, IP admits by 48%, and IP re-admits by 19% post 12 months of Chicago Northside’s interventions, thus decreasing the overall cost of care by 28% per month.

We are proud of these outcomes and their innovative model to improve access to care and increase the overall quality of life and well-being for Medicaid recipients in the Chicago Northside.

Real change happens when communities come together.

We’re proud to support Collaborative Bridges, a Healthcare Transformation Collaborative working to improve how behavioral health services are delivered on Chicago’s West Side. Funded by the Illinois Department of Healthcare and Family Services, this collaborative brings together trusted community providers, including Bobby E. Wright Comprehensive Behavioral Health Center, Community Counseling Centers of Chicago (C4), Habilitative Systems, Inc. (HSI), Hartgrove Behavioral Health Systems, Humboldt Park Health, and The Loretto Hospital, to better connect care and expand access to behavioral health services where they’re needed most.

Collaborative Bridges recently shared a new white paper on how Illinois is protecting behavioral health services amid federal funding cuts, an important read for anyone invested in community health and equity.
