Erie Family Health Centers We provide high quality, affordable care to support healthier people, families and communities.

Located at Division and Western, Erie Division Street Health Center lies in the heart of the West Town and Humboldt Park communities that we’ve been serving for more than 50 years. At Erie Division Street, we offer primary medical care for children, adults and older adults, chronic disease management, vaccines and flu shots, HIV testing, STI testing and treatment, prenatal care and women’s health services. This health center was formerly run by the Chicago Department of Public Health (CDPH). While Erie Division Street is now under our management, CDPH will continue to provide mammograms, family case management and the Women, Infants and Children program at the same location

Erie Sankofa Village Health Center is now open and ready to serve residents of West Garfield Park. Erie serves as the main primary care provider at the Sankofa Village Wellness Center, providing pediatric, adult, and prenatal medical care as well as on-site dental and behavioral health services.

As Block Club Chicago reports, West Garfield Park residents live to an average age of 69, compared to 85 for people living in the Loop. Erie and our partner tenants in the Wellness Center aim to address this gap as well as other health disparities.

It was moving to hear testimonies from community health center patients, clinicians, and hospital partners that shared how the 340B program allows them the ability to not have to choose between paying rent or their medications.

This March, we are celebrating Women's History Month and acknowledging all of the trailblazing women who have fought for equality on behalf of women today and in the future. Throughout history, have made vital contributions, often in the face of discrimination or unequal rights. Courageous women have fought for the right to vote, stood against injustice, and extended opportunities for women who follow in their footsteps.

During Women's History Month, let us honor women who have influenced and made a mark on health history, and pay tribute to those who envisioned a future where women could be doctors, clinicians, and researchers, as well as educated patients who know how to advocate for their own care.

This Black History Month, we highlight the dedicated and resilient leadership of Nicole Willis, whose steady hand and compassionate vision guide Tapestry 360 Health as its CEO. Appointed in April 2021 at the height of the COVID-19 pandemic, Willis stepped into the role with a profound sense of mission, drawing on over two decades of healthcare experience to ensure her community had a trusted, unwavering source of care.

Her journey with the organization demonstrates a rapid ascent built on competence and deep understanding. Joining Tapestry in 2018 as Director of Community Health Centers, she rose to Vice President of Operations in 2019 before assuming the top leadership position. Her leadership philosophy is rooted in servant leadership, actively fostering a supportive, inclusive, and empowered workplace culture where every team member feels valued—a principle she believes directly translates to better patient care.

Dr. Kimbra Bell Belark is a distinguished internist, educator, and advocate for health equity. As the Medical Director of the new Northwestern Medicine Bronzeville Outpatient Center, opened just four months ago, she is helping expand access to high‑quality, culturally responsive care in one of Chicago’s most historic Black communities.

A graduate of Meharry Medical College and a respected Clinical Assistant Professor of Medicine, Dr. Bell Balark has dedicated her career to improving outcomes in chronic disease, preventive care, and community health. Her leadership continues the legacy of Black physicians who have shaped medicine through excellence, service, and advocacy.

This month, we honor her impact, her voice, and her commitment to advancing health equity in Bronzeville and beyond.
