Erie Family Health Centers

Erie Family Health Centers We provide high quality, affordable care to support healthier people, families and communities.

Located at Division and Western, Erie Division Street Health Center lies in the heart of the West Town and Humboldt Park communities that we’ve been serving for more than 50 years. At Erie Division Street, we offer primary medical care for children, adults and older adults, chronic disease management, vaccines and flu shots, HIV testing, STI testing and treatment, prenatal care and women’s health services. This health center was formerly run by the Chicago Department of Public Health (CDPH). While Erie Division Street is now under our management, CDPH will continue to provide mammograms, family case management and the Women, Infants and Children program at the same location

Huge thanks to the incredible Cigna Healthcare team members who volunteered with us at this month’s food distribution at Erie HealthReach Waukegan!

As Erie marks three years of fighting food insecurity in partnership with the Northern Illinois Food Bank, moments like this remind us of what’s possible when community partners step up. Over the last year, we’ve served more than 12,000 community members through our monthly food distributions, and every volunteer helps make that impact possible.

Erie's Lending Hands for Life (LHL) program recently hosted their annual celebration of World AIDS Day. Erie welcomed patients for a luncheon, to learn from guest speakers, HIV activists Milani Varela and Jim Pickett, and to participate in an art project led by art therapist Crystal Drake.

This year's World AIDS Day theme, "Overcoming Disruption, Transforming the AIDS Response," was chosen in response to recent cuts to critical funding, disruption to HIV services and care, and heightened threats to LGBTQ+ communities around the world. This annual event is an empowering, inspiring moment of community and connection.

Getting a flu shot is important because it helps keep people from getting sick. It can stop the flu from spreading, help you miss fewer days of school or work, and make flu symptoms milder if you do get sick.

The flu shot also keeps people out of the hospital and can even save lives, especially for older adults, young kids, and people with health problems.

Erie Development Coordinator Emerson Sherbourne was swimming in gifts yesterday, all donated and wrapped by our friends at Adobe.

This is our fourth year celebrating the season together, and we’re so grateful to Erie Foundation Board member Lory Mishra and her wonderful colleagues for making Erie part of their holiday festivities once again. And a special thank-you to Adobe for also matching employee donations and doubling the impact for Erie patients. Your generosity reflects the joy and resilience of a community connected by care.

Big thanks to our friends at New Harbor Capital for their generous donation of toys for Erie patients. On December 3, their team visited Erie West Town and spent the morning wrapping and writing notes to make each gift extra special.

With 43% of our patients under 18, this support brings more than holiday cheer, it adds a little extra joy and lets our families know the community is thinking of them.

We’re grateful for who help make the season brighter for so many children and families.

As winter weather sets in, it’s important to stay prepared and prioritize safety. Key reminders include:

Limit non-essential travel during icy or hazardous conditions to keep yourself and others safe.

Dress appropriately for cold weather by layering loose-fitting, lightweight, and warm clothing.

Prepare a winter vehicle kit with essentials such as blankets, non-perishable snacks, a flashlight, and other emergency supplies.

Check in on older adults in your community to ensure their comfort and safety during extreme weather.

Prevent frozen pipes by allowing faucets to drip during very cold temperatures.
