At CommunityHealth, we understand the tremendous needs of the uninsured population in Chicago– a population that continues to grow in today’s economic climate. Thanks to an innovative and highly effective model supported by volunteerism, philanthropy and partnerships, CommunityHealth will provide 30,000 medical and dental visits at our West Town location this year for over 9,000 patients. physicians, nurses, dentists, counselors, donors (individuals, foundations, corporations, etc.) and various institutional (e.g. hospitals, medical schools, universities/graduate programs) and community partners recognize the critical role that CommunityHealth is playing in the health of those in our society who would otherwise go without care they need. While primary care is the cornerstone of our mission, CommunityHealth is a full service, appointment-based medical home providing more than 20 specialty services (from cardiology to ultrasound), health education, case management and social work (available both on-site and through formal referral partners).
