At CommunityHealth, we understand the tremendous needs of the uninsured population in Chicago– a population that continues to grow in today’s economic climate. Thanks to an innovative and highly effective model supported by volunteerism, philanthropy and partnerships, CommunityHealth will provide 30,000 medical and dental visits at our West Town location this year for over 9,000 patients. physicians, nurses, dentists, counselors, donors (individuals, foundations, corporations, etc.) and various institutional (e.g. hospitals, medical schools, universities/graduate programs) and community partners recognize the critical role that CommunityHealth is playing in the health of those in our society who would otherwise go without care they need. While primary care is the cornerstone of our mission, CommunityHealth is a full service, appointment-based medical home providing more than 20 specialty services (from cardiology to ultrasound), health education, case management and social work (available both on-site and through formal referral partners). This month is American Heart Month, and there are many things CommunityHealth is doing to promote healthy living! ❤

The Health Education Team hosted a Heart Healthy event on Saturday, inviting patients to come and learn all the different ways they can stimulate and encourage heart health! There was yoga and healthy snacks for attendees to enjoy while CommunityHealth workers talked about our Wellness West program and gave away blood pressure monitors.

Join the CommunityHealth Associate Board for a night of music, food, drinks and community -- supporting access to health care for all!

On March 6th from 6:30pm-9:00pm, CommunityHealth will be partnering with Patagonia to host "Community in Action," where every dollar raised supports CommunityHealth's work providing free, high-quality health care to the uninsured in Chicago.

At the event, you will learn more about the work CommunityHealth and the Associate Board does, all while enjoying live music, raffle prizes, and Patagonia discounts!

Every year, our Health Education team hosts a patient town hall to gain knowledge and perspectives from our patient community. Last Saturday, the Spanish patient town hall was hosted in our West Town location, where patients came to share their thoughts and suggestions.

CommunityHealth shines the Volunteer Spotlight on Izzy Mroczek as our January Volunteer of the Month!

Since joining CH in March of 2024, Izzy has contributed an extraordinary 365 nearly 100 hours over the annual expectation from volunteers. Izzy is known for her incredible helpfulness. In her downtime as an interpreter, she always steps up to fill in the gaps, asking what else she can do to help.

This year, she took on a role supporting our Outreach Team by interpreting at a Polish patient advisory council meeting at the clinic.

To celebrate their achievements, we hosted our annual Volunteer of the Year Awards! At this event, we honored 17 outstanding volunteers for their over-the-top achievements and dedication to their roles at CommunityHealth.
