My Medical Minute- Taking a minute to say thank you!

September marks the first anniversary of opening my own concierge medical practice, so I’m sharing this personal reflection today. There were moments I wasn’t sure I’d get here….

Why would I ever open my own concierge practice? Simply put, I wanted more time with my patients to direct care in the ways we see as best, as a team. I truly believe I didn’t change…medicine did. Healthcare is too rushed and transactional now. The average yearly physical in the US is 7 minutes, and that needs to change. My patient panel at my previous job was well over 3000 patients, far too many patients to provide good care to anyone. My goal patient panel of 250-300 patients means I have appointments available quickly and time to advocate for my patients' health.

I want to thank 3 special groups of people who have helped me along this journey.

First, I’d like to thank my “client” supporters, all the patients who took this leap of faith with me, both my prior patients and my newbies. Getting your care outside of corporate medicine feels good once you are there. Let’s keep working together to make your health the best it can be. And if you haven’t jumped on board yet, the time is now!

Second, thanks to all my business and practice supporters. Doctors are not trained in business, so this transition has been really challenging. I’ve learned a lot about grit, resilience, flexibility, and excel. From my informal consigliere/ business consultant/ AI guru and friend Mira (a successful pediatric dental practice owner in my town) to my partners who encouraged me to break free, I’ve had a lot of help. My lawyer, accountant, bankers, marketing team…the list goes on. Special thanks to the owners of Initium Infusion for subleasing space for my west suburban site. My incredible staff in the city deserves huge thanks! Thanks Hilario, Rebecca, Marilyn, Jayne, Tanisha, Mae, Tajuana, and Eloy! They’ve picked me up on my down days and been there to answer all my questions, or find someone who could.

Last but not least, thanks to my emotional supporters. My friends, in-laws, sister, and yes, even my priest! Anyone who been there with kind words of encouragement to simply remind me I’m a good doctor, and people will always need good doctors. Thanks to my dog Lulu; you’ll never know how much petting you on the front porch lowers my cortisol. Thanks to my 3 kids who are my motivation everyday…but often raise my cortisol. I’m so proud of the young adults you are becoming. Finally, I’m most thankful for my beautiful, smart, patient, funny, and deceptively good-business-sensed wife Anne. Thanks for letting me try this and being there to support me on this part of our journey. I love you!

My Medical Minute- VO2 Max!

Special Thanks to Jeff from CustomFit PT for testing my VO2 max recently!

VO2 max is your maximum capacity to use oxygen at your maximum effort
It is the gold standard measurement of cardiorespiratory fitness and a strong predictor of overall health and longevity

Jeff also spent 1 hour testing my musculoskeletal system, and if I'm correct, his conclusion was that if I were a horse, he'd take me out back and shoot me!

Just a little work needed on my neck, right shoulder, right sacroiliac joint, stretching my hip flexors, activating my glutes, etc
Jeff thinks all this will shave a stroke off my golf game

In all seriousness, attending to these musculoskeletal needs will not only make my golf at least more enjoyable (for me, not my playing partners) but will prevent future injury to my rotator cuff and low back, hopefully keeping me active and healthy longer

Golfer’s Elbow is officially called medial epicondylitis

Condition that causes pain and inflammation on the inner side of the elbow, where the tendons of the forearm muscles attach to the bony bump (medial epicondyle)

It’s typically caused by overuse or repetitive stress — especially from gripping, lifting, or wrist flexion motions — common in golf

Also in other activities like throwing, weightlifting, or manual labor

Common over 40 years of age as well
I've been battling it all summer after a weekend of exercise, golf, then landscaping

Symptoms include pain or tenderness on the inner elbow, stiffness, weakness in the wrist or hand, and sometimes numbness or tingling radiating to the ring and little fingers

Treatment involves rest, ice, anti-inflammatory medications
Physical therapy, and activity modification can help
A brace can help support the tendons of the forearm and create a new fulcrum for those tendons to bend, taking pressure off the elbow

Severe or persistent cases may require corticosteroid injections or, rarely, surgery
Prevention includes proper technique when lifting or swinging the club, strengthening forearm muscles, and avoiding overuse

Hit 'em straight, and work on that swing!

Helping Men, and Women, Stay in the Game Longer

Doc, how much protein do I need to eat?
The guys at the gym say I need to eat a lot more…
I actually hear this a lot from teen athletes too

Think of protein as the ultimate performance enhancing macronutrient and longevity drug
The word protein comes from the Greek word "proteios" meaning primary
Proteins are the building blocks of life
Eating enough protein can slow age-related muscle loss

The RDA for protein is 0.8g/kg, ridiculously low
So, how much do you really need? How to get it?

1.6-2.2 g/ kg is a much better goal
The math is easy on 2.2g/kg, roughly 1g per pound

Therefore a healthy 180 pound guy should eat about 180 g of protein daily
Divide it up over 4 servings, that’s 45 g per serving
In 6 oz of chicken, fish or meat, there’s 40-45 g (about 7g per oz)

What about plant protein?
It’s simply not as easy for us to use
Most is indigestible or in plant parts we don’t eat
May also lack certain essential amino acids like methionine and tryptophan

Beans and dairy like yogurt or cottage cheese can be an excellent source of protein as well

If cutting calories, never cut protein, or you may pay the price as you age

Stay Strong, Live Long- The Health Benefits of Golf
Helping men (and women) stay in the game longer

You need to know this health fact about golf for the next time your significant other becomes frustrated at your hobby

Studies have shown golfers live longer
Hard stop. Process that

For every hour you golf, you live 4.5 minutes longer….
Probably from the triad of walking, muscle activity/ balance training, and social interaction

It’s great low impact aerobic activity; waking 18 holes is 6-8 miles, probably 8 if you golf like me zigzagging the course
This leads to improved fitness, a sustained calorie burn, and improved metabolism
Getting vitamin D helps too, but put on sunscreen after a few holes
Stress, anxiety, and depression are reduced as long as you remember it’s just a game, you’re not on tour

This can all be undone by riding in a cart, having too many food and beverage stops along the way, gambling, and spending too much time at the 19th hole…
Stay healthy and hit ‘em straight out there

My Medical Minute: Rucking?!

You ever see someone, life my wife, walk the neighborhood in a weighted vest?
What are they doing? Rucking!

Rucking is what special forces use to train for their military missions

Rucking can help us non-military folks train for life and aging

It's great for muscles, bones, and balance
Rucking trains your core
It strengthens bones and helps prevent osteoporosis
Walking downhill with a weighted vest trains your brain and muscles not to fall as the muscle fibers lengthen, something we don’t usually train

Most experts recommend starting with 10% of your body weight on your back
Buying a ruck sack or weighted vest is an easy way to make sure the weight distributes evenly and doesn't cause friction on any 1 particular area as you walk/ hike/ move

My Medical Minute- NPH

Normal Pressure Hydrocephalus is not a common disease we see in primary care, but it is a potentially reversible cause of dementia
Billy Joel was just diagnosed with this a couple weeks ago and had to cancel tour dates

Causes pressure on the brain, thought to be from decreased reabsorption of cerebrospinal fluid

Classic triad, “wet, wacky, and wobbly”
Urinary incontinence is often the first symptom
Memory loss and personality changes follow
Finally, balance issues start

Fortunately, NPH is treatable, to a degree, typically with shunt placement

MMM- Alzheimer’s Disease

June is Alzheimer’s Disease Month

Alzheimer's is the most common neurodegenerative disease followed by Lewy Body Dementia and Parkinson’s

There are some new tests to screen for it and even diagnose it

My grandfather had Alzheimer’s and really, I want to know about preventing it
What’s good for the heart is good for the brain, so eat healthy and start walking and lifting weights
Brain games like Soduku puzzles and just simply reading are great for brain health
Finally, and this may not be for everyone, but having a pet has been shown to help prevent Alzheimer’s because we have to remember to care for them
The walks are good too, right Lulu?

Helping men (and women) stay in the game longer

Hey doc. A few of my friends are taking NAD. What is that?

NAD is a coenzyme in every cell in the body
It plays a roll in cellular metabolism and energy production, helping cells create more ATP (energy) at the end of the Krebs cycle

NAD decreases as we age, especially in certain tissues like the skin, and it may play a roll in diseases from fatty liver to Alzheimers

NAD can naturally increase with calorie reduction/ fasting, ice baths and saunas, and exercise

NAD is generally considered to be a safe supplement
Oral supplements and injections/ infusions exist

Please ask your doctor what may be right for you, or come see me!

My Medical Minute- Creatine!

I get asked a lot of questions about creatine, the most popular nutritional supplement in the US

It aids in ATP synthesis (energy) on the cellular level
This helps recovery after short burst muscle activity
Examples are sprints or max weight reps, like me doing 405 lbs bench presses (yeah right)

Is it safe?
It's seems really safe in studies
It can raise your creatinine, a marker of kidney function
This is normal as creatine is broken down into creatinine

I get asked about using it by a lot of teen athletes
I usually tell them to eat another chicken breast for protein and that their muscle recovery is fine
Hydrate!
But it is very likely safe when used properly

"Who gets PSA testing doc?"

How does an 80 year old former President, the most tested man in the world presumably, get screened for and diagnosed with prostate cancer
I do not know

But I do know how I screen my patients
The topic is not without controversy though
You could put 10 doctors in a room and get 10 different answers

Almost all men 50-65 (and some other ages) should get a PSA blood test periodically, watching the value itself and the rate of rise
The PSA can be influenced by an enlarged prostate, inflammation or infection, cancer, or trauma like bike riding
No more finger testing from your primary care doc
We're happy about that too
If elevated or rising rapidly, you should see a urologist, who will very likely offer to examine your prostate with a finger...

Stay in the game longer

Hey men (and ladies) out there
I’m excited for this SSLL today, testosterone!

Women in menopause have an empty tank of hormones, including T
For men, it's more like a slow lowering in the tank
If you call it manopause anywhere near your wife, she is allowed to beat you

The main symptoms of low T are poor libido and fatigue
T is best checked around 8am fasting with a blood test

Treatment-
Pros- improvement in libido, muscle strength/ mass, bone density, mood and cognition
Cons- Acne, enlarged prostate, sleep apnea, increased red blood cells, shrunken testicles

The blackbox warning for heart attacks and strokes was removed very recently

See your doctor to discuss if T is appropriate for you
