The Role:
The Visiting Registered Nurse (RN) for Home Health provides comprehensive physical and psychosocial assessment and nursing care planning for home health patients in the home setting. You will join a talented team of interdisciplinary health professionals in an environment that promotes innovation and patient-centered care. The nurse will work closely and communicate regularly with various members of the clinical team including Scheduling Coordinators, Clinical Program Coordinators and will work under the direction of Clinical Nurse Management team and the Director of Nursing. This role reports to the Clinical Manager.
The Responsibilities of the Visiting RN:
· Identify problems, counsel, and teach patients, family and caregivers appropriate nursing care skills
· Perform skilled procedures to include injections, complex wound care, catheter insertion and care, colostomy and tracheostomy, central line IV care and blood draw
· Providing services to the patient under a plan of care established and reviewed by a physician
· Exhibit excellent judgment, critical thinking, and reasoning skills as well as the adaptability necessary to fulfill the needs of and maintain the boundaries required by each patient
· Maintain accurate/current patient records for purposes of continuity of care, team communication and program quality evaluation
· Provide care in accordance with the physicians’ orders and according to acceptable standards of practice for Illinois state nursing practice act
· Exceptional communication and interpersonal skills, along with a firm dedication to customer service
The Qualifications:
· Registered Nurse with current licensure to practice professional nursing in the state of Illinois
· One to three (1-3) years of experience in nursing, preferred
· One (1) year experience in home health nursing, preferred
· One (1) year experience in high acuity setting, preferred
· Current CPR Certification
· Current driver’s license required
· Reliable transportation and current auto insurance required
· Proficient computer / laptop skills including MS Office Suite
· Excellent customer service skills
· Ability to chart in a timely fashion
· Familiarity with OASIS and EMR software (Brightree)
