Attend the Enloe Health Advance Care Planning workshop to begin the process of advance care planning (ACP).
• Learn the five easy steps of ACP.
• Understand the different ACP documents like an advance directive and POLST (Physician Orders for Life-Sustaining Treatment).
• Get the documents you need.
• Ask questions and receive answers.
• Complete a worksheet of next steps.
• Play the “Go Wish” game!
