Right now, we have providers with availability in Butler, Warren, Hamilton, Clermont, and Montgomery counties. Our team works alongside individuals and families to:

• Reduce life-limiting barriers
• Increase access to community and recreational activities
• Develop communication systems that work for the entire family

Did you know that each Ohio county has a County Board of Developmental Disabilities? These boards help individuals with developmental disabilities and their families access supports and services in their local community.

To get started, families can contact their local County Board of DD to request an intake or eligibility assessment. If eligible, the board can help connect individuals to services such as service coordination, supports for daily living, community inclusion, employment supports, and more.

Accessing DD services early can help individuals build skills, increase independence, and receive support tailored to their needs and goals.

If you’re unsure where to start or need help navigating services, Transitions BH works alongside families and community partners to support individuals through these transitions.
