Programming:
• Implement Individual Plan outcomes as specified and as part of the daily routine; document immediately.
• Provide consistent interactions with individuals and the implementation of the Individual Plan.
• Utilize the daily routine as a structure and opportunity with which to stimulate an individual’s development (mail, meals, laundry, maintenance, etc.).
• Follow the established household routine and household/activity calendar unless there is an unusual reason not to do so, and document this reason.
• Aid individuals in conflict resolution using positive techniques.
• Encourage individuals to assume responsibility for their actions and decisions.
• Implement behavior support techniques only when there is a written plan, and then do it exactly as it is written.
• Assist the individual to develop an active and full circle of friends, family, co- workers, and peers through the full use of community work and leisure activities and events.
Protection:
• Provide Personal Care Services, completing activities of daily living including, but not limited to; assistance with eating, toileting, bathing, hygiene, personal care, incontinence care and medical and health related activities, document as required.
• Provide Homemaker Services including, but not limited to: grocery and personal shopping, menu planning to individual tastes and needs, meal preparation, laundry and housekeeping services, document as required.
• Prepare and document special diets and correct food textures as ordered by the physician.
• Obtain and document vital signs, temperature, pulse, respirations, and blood pressure as ordered and as needed.
• Assist in the administration of medications or treatments as indicated by physician and following the required protocol; document the action as required.
• Familiarize yourself with the inner workings of the home and know where to find shelter, safety equipment and essential items in case of emergency.
• Report immediately to your supervisor any changes in an individual’s routine, health conditions, eating habits, bowel or bathroom habits, and changes in behavior and/or attitudes— anything about the person that just does not seem right.
• Provide staff support and supervision to the individual at the level indicated in the Individual Plan.
