🏥 Even years after Sepsis-3, sepsis reviews remain one of the most confusing areas for CDI specialists.

Often, it feels like the diagnosis is being "pushed" by AI and system alerts rather than clearly supported by the clinical story.

📉 This is why clinical foundations still matter.

While payers focus on SOFA criteria, a strong grasp of Sepsis-2 (SIRS) is still essential to interpret the record.

The challenge?

SIRS findings are pervasive, but rarely specific. A defensible record depends on whether these signs are infection-driven or caused by something else.

👇 The "SIRS Trap": Alternative Causes

We must look for persistence and clustering rather than isolated signs.

Ask yourself:

⚡ Tachycardia: Is it sepsis, or pain, anxiety, or Afib with RVR?

📈 Leukocytosis: Is it infection, or a stress response, steroids, or recent surgery?

📉 Hypotension: Is it distributive shock, or simple dehydration and medication effects?

🫁 Tachypnea: Is it respiratory failure, or metabolic acidosis and pulmonary edema?

🔥 Fever: Is it the source, or a drug fever, transfusion reaction, or malignancy?

🖼️ Beyond the Checkbox

"Infection + one abnormal vital sign" is rarely enough for a compliant query. The real work is in the synthesis of the whole picture.

🔍 We must evaluate the timing and trajectory of the illness, the response to fluids and antibiotics, and trends in lactate and culture data.

When you combine those with imaging results and the provider’s reasoning across the chart, you build a story that stands up to scrutiny.

AI can surface patterns, but it cannot determine causation.

🤖 Pairing AI signals with a disciplined clinical lens is what keeps queries valid, compliant, and defensible.

📞 If you looked at this photo from Dr. Tarman Aziz’s Internal Medicine residency at the University of Oklahoma Health Sciences Center in Tulsa, you would probably notice the state-of-the-art corded phone first.

🩺 But for him, this photo represents a completely different era of medicine.

📄 Back then, medicine lived out of paper medical records. Dr. Aziz remembers the "coder queries," those nuisance documents, three-hole punched and tucked into the chart so they stuck out like an offset bookmark. They were designed so you could not miss them, but when you are a busy resident, it was remarkably easy to "overlook" them while rushing to the next patient.

✍️ He certainly did not think his handwritten notes were "awful" at the time, and he definitely did not think he would eventually make a career out of them.

It is funny how things work out.

🔄 Fast forward to today, and our Founder & Chief Executive Officer, Dr. Tarman Aziz, is leading Clinical Documentation Integrity (CDI) initiatives in hospitals and teaching the complexities of documentation to everyone from new learners to seasoned professionals.

At CDIQ Consulting, we have seen him go from dodging those "bookmarks" to showing others why they are the key to clinical accuracy.

📌 We are so proud to see our Founder & CEO, Dr. Tarman Aziz, MD, CCDS of CDIQ Consulting featured in the Journal of American Health Information Management Association (AHIMA) with the newly published article, “Enhancing Inpatient Clinical Documentation Integrity (CDI) Outcomes: Best Practices in Documentation and Coding.”

This article provides an overview of the key themes shared during Dr. Aziz's AHIMA25 workshop, using case-based inpatient scenarios to examine five high-risk conditions, sepsis, acute kidney injury (AKI), end-stage renal disease (ESRD), congestive heart failure (CHF), and malnutrition, while emphasizing how stronger documentation, coding defensibility, and disciplined clinical reasoning can better withstand audit, denial, and clinical validation scrutiny.

It also reinforces one of the session’s central messages that as artificial intelligence (AI) becomes more embedded in CDI and coding workflows, organizations must pair innovation with strong governance, provider accountability, and human oversight.

📈 This recognition reflects the growing importance of strong documentation, coding accuracy, and disciplined CDI practices in today’s evolving healthcare environment.
