📄 Step 1: Email your local clinic to request the Sports Physical form.
✍️ Step 2: Fill it out and bring it with you when you visit Integrity Urgent Care for your physical.
📅 Step 3: After your visit, return the signed flyer to your Coach or Athletic Director by September 30, 2025.
