Grand Hospice is growing, and we’d love for you to grow with us! We’re looking for a compassionate, skilled Hospice RN Case Manager who is ready to make a meaningful difference every day.

As an RN Case Manager, you’ll play a vital role in providing exceptional, dignity-centered care to patients and families across Fort Worth, TX and surrounding areas. You’ll manage a small, quality-focused caseload, coordinate with our interdisciplinary team, and offer the clinical leadership that defines outstanding hospice care.

When a serious illness progresses, families are often left wondering whether it’s time to consider hospice. At Grand Hospice, we’re here to bring clarity and compassion to that moment.

Common diagnoses that may lead to hospice eligibility include:

🤍 Advanced cancer
🤍 Heart disease (such as CHF)
🤍 Lung disease (such as COPD)
🤍 Dementia/Alzheimer’s
🤍 Kidney or liver failure
🤍 Neurological conditions such as ALS, Parkinson’s, or stroke-related decline
🤍 Significant frailty or overall decline

However, eligibility isn’t solely based on diagnosis. How the illness affects daily life, comfort, and quality of living are also taken into great consideration.
