After some sincere thought, much consideration, and the changing climate of the long-term care industry, The Pye family decided to close our business after 54 memorable years. Fairfax Place is a long-term care facility providing a range of services, including skilled nursing care, respite care, hospice care, occupational/physical/speech therapy, and catering services.

We are looking for dedicated, resident-focused, take-charge Licensed Practical Nurses with at least one year of experience in a nursing facility. Knowledgeable of the federal and state licensure regulation for nursing homes, including care planning and rehabilitation care. Experience in assisting with assessments, analysis, planning, implementation, evaluation, and documentation of clinical nursing care.

Qualified candidates will be a graduate of an accredited LPN program and hold a current license to practice as a registered nurse in the state of Ohio. Have the ability to communicate effectively verbally and in written form. Candidates must be knowledgeable of or participate in the requirements of the position including but not limited to: professional requirements, care plan/assessment functions, charting, documentation, medication administration, nursing care functions (admission, discharges, transfers, technical aspects, interdisciplinary team functions), personnel functions, resident rights, safety& sanitation, and staff development.

The State Tested Nursing Assistant or Certified Nursing Assistant provides routine daily nursing care to assigned residents in accordance with established nursing care policies and procedures under the direction of the licensed charge nurse. Qualified candidates should be able to observe patient conditions, measure and record food/fluid intake/output, vital signs, and report changes to charge nurse.

We are currently seeking a full-time experienced cook to prepare large quantities of food following standardized recipes and production guidelines.

The qualified candidate must know methods of food preparation, cooking times, cooling and reheating, and portion sizes to ensure food is prepared in a prescribed manner. Must be flexible and willing to work every other weekend, holiday, and fill in as needed. Must have and use sanitation practices for work areas, equipment, and utensils. Adhere to weekly/monthly cleaning schedules as posted by management. Ability to complete any task requested by supervisor or member of the Fairfax Place management team.

Fairfax Place is a family owned and operated facility that prides itself on the quality care we provide our residents. We currently are looking to expand our team!

Fairfax Place is in need of a FULL-TIME and PART-TIME Dietary Aide to work four (4) days/week and every other weekend. The qualified candidate will be responsible for maintaining a clean and safe kitchen environment and following sanitary guidelines. The Dietary Aide is responsible for food preparation, assist cook in preparing meals, perform dishwashing/cleaning procedures, sweep/mop floors as directed, serving residents as instructed, and other duties assigned by the Dietary Manager.

The ideal candidate will have:

The Dietary Porter maintains the cleanliness of assigned dietary area(s) and equipment. The Dietary Porter will be responsible for lifting, stacking, and storing equipment/food. Dishwashing is a primary duty, but also sweeping, mopping, and the removal of trash. The porter will uphold the mission, vision, values, as well as customer service standards of Fairfax Place.

Required:
Must be able to read and write English.

HS Diploma or General Educational Development (GED). Physical Requirements: Will stoop, bend, and ambulate intermittently during the day. May need to sit or stand for extended periods. See in the normal visual range with or without correction. Hear in the normal audio range with or without correction. Ability to push/pull/lift 100-300 pounds. Requires exposure to chemicals and cleaning products. Requires exposure to warm and cold temperatures.

An ideal candidate for this role would be responsible for planning, implementation and documentation of social activities intended to contribute to a high quality of life for our resident. This position will also develop and manage a comprehensive volunteer program that enriches the lives of our residents and brings joy and fulfillment to the individual volunteers. Additionally, all programming will be of high quality and marketed in a manner that attracts the respect and attention of our families, our staff and the general community.

Responsibilities
Take full responsibility for the planning and implementation of the facility’s resident activities function, which includes:
1. Insuring that necessary and appropriate documentation is maintained for all residents to meet regulatory and facility requirements
2. Supervising and support all facility activities staff
3. Developing and maintaining a direct relationship with family members of all facility residents
4. Preparing families and residents to complete bi-annual Resident-Family Survey and Resident Satisfaction Survey (both sponsored by the state of Ohio)
5. Developing a system to recruit, train, schedule and recognize facility volunteers
6. Representing the Fairfax Place at community based functions
7. Assisting the administrator with investigating and resolving family and resident complaints
8. Meeting with new residents and family member to assess the resident’s activity preferences and needs
9. Developing a formal activities marketing component that encourages residents to participate in the programming and reinforces in the mind of our staff, family members and the general public that Fairfax Place is a great place for residents to live
10. Managing facility relationships with ancillary physicians and coordinate resident schedule for ancillary doctor visits
11. RELATIONSHIP BUILDING. An ideal candidate will take responsibility for being proactive in assure strong, positive relationships with residents, staff, family members and all others involved in Fairfax Place community

Qualifications

1. Excellent written and verbal communication skills
2. Must be outgoing, friendly and capable of speaking in front of groups
3. Energetic and capable of working with seniors/ elderly population
4. Flexible and able to attend community functions in the evening and on weekends
5. Certified Activities Coordinator or equivalent experience (preferred but not required)
6. CTRS or Occupational therapy license (preferred but not required)
7. Computer savvy
8. Keep a can-do mindset and find creative solutions to potential issues
9. Networking skills (prior volunteer recruitment experience preferred but not required)
