For over 40 years Neighborhood Family Practice (NFP) has been providing health care on the Near Westside of Cleveland and Lakewood. NFP has a mission and vision dedicated to improving access and eliminating barriers to care. With seven locations, NFP services include primary, preventive and supplemental health services, as well as behavioral health, pharmacy and midwifery.

A message from our President and CEO, Domonic Hopson
As we close out 2025, I want to begin by thanking our incredible staff and the communities we are honored to serve. Your trust, dedication, and care for one another made this year of growth and change possible.
Health care continues to bring challenges, but our shared commitment to community-based care kept us moving forward—together.
This year, we opened the W. 130th Community Health Center, made strong progress at our future Clark Fulton location ahead of a March 2026 opening, and moved closer to launching our W. 130th drive-through pharmacy. Each milestone reflects our promise to expand access and meet people where they are.
To our donors, partners, and supporters—thank you for walking alongside us. And to our staff, thank you for showing up every day with heart and purpose.
Here’s to 2026, filled with partnership, growth, and continued care—and thank you for being part of the Neighborhood Family Practice legacy. 💙

Our Story

Neighborhood Family Practice was founded in 1980 in response to a concern that residents of Cleveland’s near west side lacked access to primary health care. A Federally Qualified Health Center (FQHC), NFP focuses on providing high quality primary care in a medical home setting.

As a community provider, our five convenient locations offer same day appointments. The NFP service area spans twelve neighborhoods across Cleveland’s west side and is comprised of a very diverse mix of residents.

NFP accepts Medicaid, Medicare, most commercial insurance plans and offers a financial assistance program (sliding fee scale) for the uninsured.

Neighborhood Family Practice is a recipient of the Health Resources and Services Administration (HRSA) and each of its locations are covered by the Federal Tort Claims Act (FTCA).
