Referral Valet ReferralValet™ is a subscription based referral community (network) that serves professional care managers in Social Services and HealthCare.

ReferralValet helps not only in the initial referral process but also in the much needed care coordination as a compliment to services provided in Nursing Homes. Greater Cleveland Hospitals which include the renown Cleveland Clinic and University Hospital Systems are re-designing patient care for better outcomes. RV has recently been selected to work with University Hospitals to tackle this issue

If hospitals don't stop patients from being readmitted, they will pay. Starting in October 2012, Medicare will begin withholding payments to those that have higher-than-expected 30-day readmission rates for patients suffering from pneumonia, heart attack or heart failure.
