Macomb County Adult Day Service Welcome to Macomb County Adult Day Service - a respite program that provides a combination of both Social Model Day Service and Medical Model Day Service.

Thank you for your interest in the Macomb County Adult Day Service program. The program is located in the VerKuilen Building at 21885 Dunham Road, Suite 6 in Clinton Township and exists through the collaborative effort of the Macomb County Community Services Agency (MCCSA) and the Martha T. Berry Medical Care Facility (MTB MCF). Adult day care offers respite opportunities to caregivers. The National Institute on Aging reports that caregivers are especially susceptible to infectious disease due to the never ending demands placed on them when caring for a loved one. Regular respite for the caregiver is needed in order to stop or reduce stress related illnesses. The Adult Day Service program is designed to provide medical/rehabilitation services and social interaction to senior adults residing in Macomb County who have physical or mental disabilities. Restorative therapy, nursing and medical monitoring, dietary and personal care needs are readily available due to the linkage with the Martha T. Berry Medical Care Facility. The supervised support services may delay the need for nursing home placement. Door-to-door transportation is available to those participants residing in Macomb County. Participants who elect to use program transportation are picked up at their homes in the morning and returned home at the end of the day by a lift-equipped vehicle provided by Macomb County Community Service Agency’s van/bus service. During the day, the participant will engage in a wide variety of activities plus Physical, Speech and Occupational Therapies if requested and ordered by the participant’s physician. A noon meal and morning and afternoon snacks are provided. Personal care services such as grooming, hygiene, and toileting are an important feature of the program. Besides adult day care, 24-hour respite care for up to two weeks is available on a scheduled basis. During the day, the participant enjoys adult day care activities; at night respite staff assures that individual needs are met. The link with Martha T. Berry Medical Care Facility assures that emergency support staff is available at all hours. A participant must have a current (within three months) chest x-ray, which screens for TB and recent history and physical information from their primary care doctor. A list of all prescribed and over the counter medications he/she uses must also be provided. Adult Day Service is a cost-effective alternative to other forms of health care, such as home health and nursing home care. Discounted rates are available for multiple day attendance. Funding assistance may also be available through Veteran’s Administration and the Area Agency on Aging 1B to those who qualify. Please contact the center for more details. Scheduling is flexible and determined by family/participant and intake staff. The program is open Monday through Friday from 8:00 a.m. to 5:00 p.m. Extended hours are available with prior arrangement.

GOALS OF THE PROGRAM To improve and maintain the highest possible quality of life and help the participants remain in their own home environment. To minimize or eliminate the physical handicaps of the elderly. To improve or maximize the effectiveness of participant’s cognitive skills. To improve compliance with medical management. Eliminate barriers hampering the management of participant’s personal care. Afford desirable respite between the elderly patient and the caregiver. Encourage the development and/or maintenance of healthy nutritional habits. To promote education for the relative/caregiver in order to enhance their abilities to meaningfully function with the participant/patient. To improve the psycho-social skills of the participant. To constrain the rising costs of health care. SERVICES OF THE PROGRAM Physician ordered rehab services: skilled and restorative therapy. Medical monitoring and medication management by ADC nurse. Personal care given by certified nurse aides: bathing, hair care, nail care, podiatry Socialization and recreation activities Transportation throughout Macomb County to and from the Center Morning and afternoon snacks and a well-balanced lunch
