Before heading to your first WIC appointment, you’ll want to make sure you have proof of identity, proof of residency and proof of income. A few examples of each are listed below. For the full list of examples, please visit WIC.Mo.Gov.

Proof of identity:
• Infants and children: immunization record, birth certificate, hospital records.
• Adults: driver license, passport, school ID card, military ID card, naturalization record.

Proof of residency:
• Most recent or current utility bill or personal bill mailed to residence.
• Voter registration card.
• Pay stub that displays name and physical address.

Proof of income:
• Current pay stubs (paper or electronic) noting the pay frequency (e.g., weekly, bi-weekly).
• Unemployment letter or notice.
• Recent Leave and Earnings Statement (LES) for military personnel.
• Foster child placement letter or foster parent award letter.

*Follow the FDA's Recommendations for drugs on the "flush list" and they should be flushed immediately.

*For Non-Flush Medications-Mix medicines with an unappealing substance such as dirt, cat litter or used coffee grounds, place the mixture in a container such as a sealed plastic bag, throw away the container in your trash at home and delete all personal information on the prescription label.
