PCN gives full-time support to adults 18 and older on the HAB, ID, and BI waivers through our Host Home program. This service provides a safe, family-like home where people get help with daily life, joining in the community, and becoming more independent. Host Home Services: We match individuals with compassionate providers who open their homes and hearts to offer personalized 24/7 support. Community Engagement: From parades and baseball games to craft nights and cookouts, we believe in creating joyful, inclusive experiences. Growing Services: We’re always looking ahead—our team is actively working to launch Dayhab Services and Supported Employment as well as other supports that meet evolving needs.

Iowa is proposing significant updates to the disability waiver system. As part of the HOME Project, Iowa HHS plans to transition from six disability-based HCBS waivers to two streamlined waivers. The stated objectives are to simplify services, support people in remaining in their homes and communities, and improve access to needed supports.

Stakeholder input is encouraged. Questions, comments, and concerns may be submitted to the Long-Term Services and Supports (LTSS) Policy Team:

Service doesn’t always look big or bold; often, it’s found in small, meaningful acts that make a real difference in someone’s life.

Today, we invite you to reflect and engage: What does service look like to you, and how are you honoring the Day of Service? Whether it’s volunteering, lending a helping hand, or offering kindness to someone in your community, every act matters.
