PCN gives full-time support to adults 18 and older on the HAB, ID, and BI waivers through our Host Home program. This service provides a safe, family-like home where people get help with daily life, joining in the community, and becoming more independent. Host Home Services: We match individuals with compassionate providers who open their homes and hearts to offer personalized 24/7 support. Commun

ity Engagement: From parades and baseball games to craft nights and cookouts, we believe in creating joyful, inclusive experiences. We’re excited to be part of the Together We Can Conference 2026—and we’d love to see you there with us!

Hosted by ASK Resource Center, this statewide conference brings together individuals with disabilities, families, and professionals for a day of connection, resources, and learning.

PCN will be there supporting, engaging, and connecting with our community throughout the event.

This year’s keynote speaker, Chris Ulmer, brings a powerful message of acceptance and inclusion through his work with SBSK (Special Books by Special Kids). It’s a message that closely aligns with what we value at PCN.

Melissa had an incredible time at the Special Olympics in Iowa City earlier this month! 🏀

She played in two games—starting strong with a win in her first game (black jersey) and giving it her all in the second (white jersey).

Her team’s hard work and determination paid off, earning them 2nd place overall and bringing home a well-deserved medal! 🥈
