RN License State of Idaho or Compact.
BSN degree preferred however ASN accepted.
2 years experience in Home Health or related in-home care nursing services preferred .
2 years Experience in Case Management necessary.
RN Case Managers (RNCM) work in the patients home, and a variety of facility settings.

The RNCM oversees the care of patients, managing the direct care provided, helping with treatment planning, through orders from Doctors, Nurse Practitioners and Physician Assistants and support all other staff involved in the plan of care: PT, OT, ST, and MSW as you supervise the plan of care.

Communication: Case managers should be able to communicate effectively with their manager of clinical practice, other nurses, and therapists, patients and their families, and the administrator.
RNCMs must have strong decision-making and communication skills in a fast-paced, complex, and multifaceted environment. Must have strong organizational and delegation skills.
Political and cultural sensitivity. As case managers work with patients and staff from diverse backgrounds, it is important for them to be aware of political and cultural differences, serving all patients and dealing with all employees in an unbiased fashion.
