Quail Ridge Rehab and Living Center is a 118 bed Skilled Nursing Facility located on Stateline RD Colcord, OK directly beside Siloam Springs, AR. We offer rehabilitation services, providing Physical, Occupational, and Speech therapy and we provide long term care. Quail Ridge has a secured memory care unit. We have the ability to provide respite care and hospice care within our facility. Additionally, our facility provides transportation for our residents to help with transfer to appointments and activities.

Thank you to our Marketer, Lisa Greathouse. She did the shopping & delivery! Lisa is very involved with the community & we love that!

As a member of the Rotary Club here in Siloam Springs, I wouldn’t have known about this program if it wasn’t for attending that meeting. Rotary really does come together for the community!

Quail Ridge Living Center is a 118-bed nursing home located on Stateline Road Colcord, OK behind the casino. We offer skilled and long-term care services. Our skilled therapy team has the knowledge to progress residents to their highest level of function. Each resident receives an individualized plan of care to help them achieve goals for discharge or even long-term goals if staying in our facility. In our facility we offer the following nursing services:
