Quail Ridge Rehab and Living Center is a 118 bed Skilled Nursing Facility located on Stateline RD Colcord, OK directly beside Siloam Springs, AR. We offer rehabilitation services, providing Physical, Occupational, and Speech therapy and we provide long term care. Quail Ridge has a secured memory care unit. We have the ability to provide respite care and hospice care within our facility. Additionally, our facility provides transportation for our residents to help with transfer to appointments and activities.

Our skilled therapy team has the knowledge to progress residents to their highest level of function. Each resident receives an individualized plan of care to help them achieve goals for discharge or even long-term goals if staying in our facility.
