Why Join Us? As a critical access hospital, we provide personalized care in a close knit community. This role offers the opportunity to shape a vital service line and make a meaningful impact on patient recovery.

Key Responsibilities
• Coordinate all Swing Bed admissions, transfers, and discharges
• Serve as the primary liaison between patients, families, providers, and care teams
• Complete utilization reviews and ensure regulatory compliance
• Track outcomes, maintain documentation, and support quality improvement
• Promote the Swing Bed Program to referral sources and community partners

Qualifications
· RN or LPN preferred; relevant clinical or case management experience preferred
· Strong communication and patient-advocacy skills
· Knowledge of Medicare Swing Bed regulations a plus – training will be provided
· Ability to work independently in a rural hospital setting
· Excellent communication and coordination skills
· A heart for rural healthcare and patient-centered service
