Upper eyelid surgery is one of my top 3 favorite procedures! It is transformative but yet preserves YOU. It is refreshing, natural yet you still look like you. The eyes just seem *lighter* without any obvious answer as to why; you look *better* but no one is able to put a finger on why. It flies under the radar but the improvement is undeniable. It doesn’t change the shape of the eye and the incision is hidden right along our existing eyelid crease!

About 4-5 days of downtime pending bruising and swelling.

The first 2 photos are on post operative day 1. The others are day of surgery. Suture removed on day 5 or day 6.

This patient followed all the “rules” and hardly has a bruise to show for it.

It takes me right back - I still feel like I am sitting in Chemistry class all over again, watching the news, watching as the 2nd plane crashed, as the towers fell. The fear, the terror, the life lost, the ashes, but also the unity, the love, the resilience & the bravery. The true American spirit.

What can filler do for you?
….
As we age, there is no doubt we experience change at multiple levels - skin (thickness, elasticity, collagen), fat (volume loss, fat pad descent), ligaments (weaken), & bone (loss of vertical height, thinning). These in combination create appreciable changes that can be hard (at times) to pinpoint in terms of what “changed” but “it’s just different” - here is an example of how filler is used effectively & efficiently to improve on those subtle cues - starting here with the chin.

Using 1 syringe of strategically placed filler, we were able to restore a youthful taper to the lower 1/3 of the face (diminishing a squared off appearance or triangular heaviness that often comes with aging), improve her projection, & enhance her OWN features by simply reframing.
….
Certainly with some additional filler used judiciously we can continue to enhance result but one must caution that too much filler anywhere including here can not only widen & distort the face but also artificially age someone in an unnatural way.

Let’s talk DURATION! Love this graphic from regarding onset of action, peak action & then the dissipation of neuromodulator ( or )with a gradual return of muscle function. ...
It is NORMAL after 2-3 months or 8-10 weeks to see a gradual return of movement. Some patients get a little longer, some get a little shorter - there is some variability depending on the individual- but overall this depicts a generally normal timeline. ...
As is often discussed with patients in clinic, generally speaking every 3-4 months (or once a season) is all that is needed. Also, those who know me hear me say this all the time, but after consistent use for a period of time at regular intervals, what we see in practice is that you can go *longer* between appointments as the muscles “forget” how to do such heavy lifting (think of it in terms of working out ones 💪🏻 - the less you work out, the less you can lift). ...

The buccal fat pad (one of which is pictured here) & its removal has recently become more en vogue as we have seen a heightened interest in a more defined cheek & cheekbone complex....
It is incredibly rewarding for both the patient and myself when performed in the right person. Also, make sure you choose wisely - as pictured in a few images here - many very important structures in this region (namely parotid duct, facial nerve branches, & important vessels)....
The procedure is commonly performed as part of facial contouring and frequently in conjunction with liposuction, chin augmentation and the like. This gives an overall beautiful natural result while still preserving who YOU are....
While buccal fat pad removal yields results, it must be approached with caution. The majority of patients who ask about this surgical procedure are not candidates. The reason why lies for the most part in considering how each particular patient will age. If done aggressively, to really reach for that “wow” sculpted result, you may run into the fact that in a few years this area will appear deflated, sunken & artificially aged ahead of course. Not the case for everyone though so worth a conversation!...

I have this conversation all the time with patients (especially my surgical & filler patients) regarding retaining ligaments in the face. Having an understanding of these is incredibly important in terms of why we see some of the changes we do with the face as we age. Granted, this is something for me to know inside & out to treat you best, but y’all know how much I love an educational opportunity! ...

Love being able to contribute to the progress of our field alongside so many greats ! My recent book chapter is published in the new textbook “Complications in Minimally Invasive Facial Rejuvenation: Prevention and Management” focusing on complications due to filler, keys to prevention & management in the instance they occur....
As we see an astronomical increase in filler use as well as a diverse field of those performing injections, it is critical that you seek someone proficient in not only the injections (that’s the easy part!), but also in preventing, recognizing & managing issues that may follow. Your face will thank you!!

Early results from a - she is loving these results & so are we!...
For some patients, depending on their goals & their individual anatomy, a Necklift by itself is an option. This helps to achieve a more defined neck & jawline, as seen here in this profile view, with limited incisions....
She still has some swelling & the ears are a tidge swollen as well, but her incision is nowhere in sight. These results will continue to refine in & improve as well....

Tuesday in the OR: Today was all about the eyes 👀 ! ...
We carry so much in our eyes - emotion, culture, personality, expression, family - it is incredibly important to keep you YOU! I think just adds refinement, adds definition, & a little zhush to such a “window to our soul.” ..
Eyelid surgery is transformative (think refreshed, less tired appearing) yet (when done well) preserves your identity. ...
Captured here is a post operative photo of an upper eyelid blepharoplasty, lower transconjunctival blepharoplasty with conservative fat excision (think under eye bags) & skin excision to further sharpen the results. Another adjunct not pictured here that makes a beautiful result even better is a peri-orbital / around the eyes fractional CO2 laser (this can be done alone or in conjunction with surgery). The laser further refines & “shrink wraps” the lower lid skin. ...
Equal amounts of skin & fat are taken from both sides barring any significant asymmetry. This is the left eye only (the skin is a little challenging to unfurl at the end of the case)....
Outpatient surgery, 4-5 days of social downtime, sutures in the upper lid are removed on day 5!
