PDHC is hiring for the following positions:
* Community Health Advocate - The Community Health Advocate’s role is to improve health outcomes of women during pregnancy and infants early in their development to reduce pregnancy complications, infant mortality rates, and maternal mortality rates. The Community Health Advocate is a support to increase clients’ usage of preventative care, enhance provider-patient communication, and provide a link between clients and their health and social services resources.
* Connections Lead Coordinator - The Connections Lead Coordinator is responsible for overseeing the Connections Team as well as extending compassionate and timely direct client care to include crisis intervention via 24-hour hotline and online consultation. Connections services are intended to meet clients at their point of need, often as their initial contact to PDHC. Along with the Coordinators, the Lead Coordinator also assists center and administrative staff with client outreach, multi-center scheduling needs and organizational communication.
