We believe in providing quality healthcare at your convenience! We utilize a direct primary care model versus the typical fee-for-service insurance billing, meaning that we eliminate insurance companies standing between you and the exceptional care you deserve.

At Columbus Health & Wellness, an affordable, flat rate monthly payment will cover most services offered such as annual physical exams, acute care visits, chronic disease management, hormone replacement therapy and direct contact information for your provider after hours via phone, text, or email.

While this membership style care model is a great opportunity for many, we are happy to see you whether or not you are a participant of our monthly membership. Walk-ins and acute care visits are welcomed at a flat rate fee due at the time of service.
