LPN takes an active role in direct resident assessment and care
Formulates individualized nursing care plans
Attends to residents daily to observe and evaluate each resident’s physical and emotional status; Implements a change in the course of care as needed
Administers and documents medication and treatments per the physician’s order and accurately records all care provided
Supervises and coordinates nursing personnel in providing direct resident care
Takes an active role in communicating, teaching and training with residents, families, and staff members

Assist residents with performing basic tasks such as eating, dressing and toileting when necessary
Respond immediately to calls from patients for assistance or treatment and alert medical staff to pending emergency situations
Encourage all residents to get exercise and participate in scheduled activities
Engage in housekeeping tasks such as replacing linens on beds, cleaning patient rooms and removing and replacing trash bags
Work with dining room personnel to ensure that all residents get the correct meals
