IMPORTANT UPDATE!! The Ohio Department of Health is currently experiencing IT issues, which have temporarily taken the Ohio Amblyope Registry registration form offline.

We’re working closely with ODH to get everything back up and running as soon as possible and will post an update once the form is available again!
