55-year-old client with a history of complications following cosmetic breast surgery. She experienced tissue necrosis that required several months of wound care before the area was stable enough for treatment.

She presented for ar**la restoration once healing was complete. These images show her fresh results after the first session. Additional refinement and color balancing will be completed at a scheduled touch-up appointment as the tissue continues to heal and reveal its response.

52-year-old client diagnosed with left breast cancer who was treated with bilateral mastectomies, immediate DIEP flap breast reconstruction, and chemotherapy. Following final pathology, it was determined that radiation therapy was required on the left DIEP flap. Breast reconstruction was performed by Dr. Tiwari at Midwest Breast and Aesthetic Surgery.

These images show her results after a touch-up session using Ni**le Fairy Inks as part of her ar**la restoration process.

When working with radiated tissue, it’s critical to understand that it behaves differently than non-radiated skin. Radiated tissue can heal lighter, darker, or cooler in tone, and ink implantation can be more challenging. For this reason, treatment is approached cautiously and methodically. The radiated side is always addressed first, using the same pigment blend for both sides and making adjustments during the touch-up phase as healing reveals how the tissue responds. This type of tissue cannot be predicted—it must be observed and respected throughout the healing process.

She was referred for ar**la restoration by Dr. Elswick to complete the reconstructive process. Paramedical tattooing was used to enhance color, dimension, and overall symmetry following surgery.

70-year-old client whose pathology report following a breast reduction revealed both invasive ductal carcinoma and invasive lobular carcinoma. She was treated with bilateral mastectomies and breast reconstruction with ni**le–ar**la grafts.

She had previous medical tattooing that faded over time and left behind scar tissue. These images show her results after a corrective touch-up session. The goal of treatment was to restore color continuity and soften the appearance of scar tissue using a custom blend of Ni**le Fairy Inks. The client was very pleased with the outcome.

50-year-old client with right breast cancer treated with chemotherapy and bilateral mastectomies by Dr. Natalie Jones at OhioHealth. She later underwent DIEP flap breast reconstruction performed by Dr. Kale of Midwest Breast and Aesthetic Surgery.

Her 3D ar**la restoration was completed using a custom blend of Ni**le Fairy Inks to achieve soft, natural color and dimensional shading tailored to her reconstruction results.

Did you know it's not the responsibility of the health dept (In my area of Columbus, Ohio) to ensure that a tattoo artist is competent? It is 100% not.
The health dept issues licenses and does maintenance checks to make sure the artist has completed some type of "training ". The"training" used to be a signed letter from your apprenticeship and now a certificate is all that's needed and online ones are accepted. You definitely cannot learn how to tattoo online.

They verify your studio has adequate lighting, a non permeable floor surface, hot water, inks and needles that aren't expired, current blood borne pathogens, 1st aid, and CPR certificates.

That is it.

There is no exam written or practical.
It is not their job to determine if the artist is knowledgeable, competent, or artistically skilled. That's what apprenticeships do, which unfortunately are becoming extinct.

"Well, everyone has to start somewhere ". As a nurse who was not adequately trained to tattoo, I can confidently tell you that's wrong. Start on fake skin, then yourself, then friends and family. Get really good at tattooing on healthy skin before you consider taking on the huge responsibility of medical tattooing.
Survivors should not be a new artist's guinea pigs. There is a human being at the tip of your needle and they need to be cared for and respected.
