Do you have a loved one you're looking to honor this holiday season? We invite you to add their name to this year's Season of Remembrance online honor roll.

Those who choose to donate $250 or more to our Hospice Program between now and Dec. 31 will receive a special hand-blown glass keepsake ornament from our very own Jeff Lamy, former league of NH Craftsmen juried member.

We will continue to hold all our beloved community members in our hearts and memories throughout the season.

Remembering loved ones who have passed away is an important tradition for Granite VNA. Each winter we take pride in providing an opportunity for our community to pause during a typically busy time of year to remember those no longer with us.

We’re honored to receive a generous grant from the Fraternal Order of Eagles Concord #613 Auxiliary in support of our hospice patients living with dementia and Alzheimer’s disease. 💜

These funds will help us create specialized activity kits that include resources to encourage reminiscence, reduce stress, and foster more meaningful engagement between patients and their loved ones.

Our volunteers are eager to begin rolling out this program at The Birches of Concord, a memory care facility.

Please join us in congratulating Derek, RN, and Kassandra, RN, on successfully completing our nurse residency program! 🎉

Over the past year, Derek and Kassandra have worked as home care case managers while participating in a comprehensive orientation and training program for new graduate nurses. Their colleagues and patients consistently describe them as dedicated, compassionate, curious, and eager to grow — qualities that make a profound difference in home-based care.

A heartfelt thank you to their preceptors — Jennifer, Chelsea and Rachel — for your guidance, mentorship, and support throughout their journey.
