We are currently seeking a licensed Physical Therapist (PT) and/or Physical Therapist Assistant (PTA) to provide in-home services for our growing home health agency in SC and NC. This role is ideal for experienced clinicians seeking flexible scheduling and consistent referrals.
The contracted clinician will be responsible for performing new client evaluations and/or routine skilled therapy visits in a home health setting. Services are provided in accordance with physician orders, agency policies, and applicable state and federal regulations.
Compensation is paid per completed visit (1099 Independent Contractor)
Responsibilities
Perform comprehensive initial physical therapy evaluations (PT only)
Provide routine skilled therapy visits in the patient’s home (PT or PTA, per scope)
Develop, implement, and update individualized plans of care
Accurately document all evaluations, treatments, and patient progress
Communicate effectively with physicians, agency staff, patients, and caregivers
Ensure compliance with all South Carolina and North Carolina licensure requirements and agency policies
Educate patients and caregivers on treatment plans, safety, and home exercises
Qualifications
Current South Carolina and North Carolina PT or PTA license in good standing
Minimum 1 year of clinical experience (home health preferred)
Knowledge of home health regulations and standards of care
Strong clinical judgment, communication, and documentation skills
Reliable transportation and valid driver’s license
Professional liability insurance (or willingness to obtain)
Ability to work independently and manage a flexible caseload
Compensation Range: $85.00 - $95.00 per visit.
