Bridgeway Home Health is locally owned and operated within the state of Georgia and currently services Rockdale and Newton counties. Taking care of patients at home can be a challenging and daunting task. Home is the best place for recovery if we keep certain factors under control and Bridgeway understands the needs of the local communities that we serve and is focused on providing the highest quality home health care with competence and compassion. Our professional and caring clinicians will help you and your family understand your illness and how to "bridge your way to better health." Your individualized plan of care is developed by our team in conjunction with your physician to maximize improvement in your health. Your recovery and rehabilitation is our primary focus so that you can maintain your independence and live your life to the fullest. Home health may be necessary for your recovery following a stay at a hospital or rehabilitation center, post-op care, if you have been diagnosed with a new illness or disease, significant change in your medical condition, status, or medications, if you have fallen recently or at risk of falls, if you have a chronic disease requiring clinical management (CHF, COPD, MS, Diabetes, or Alzheimer's), IV therapy, or wound care. Bridgeway provides skilled nursing, physical therapy, speech therapy, occupational therapy, home health aide, and medical social work.
